Efeitos do treinamento neuromuscular na propriocepção após reconstrução do ligamento cruzado anterior: Uma revisão sistemática

Resumo: Objective: To analyze the effects of neuromuscular training on proprioception after anterior cruciate ligament reconstruction (ACLR). Methods: Systematic review, described based on the PRISMA flowchart, including randomized clinical trials in Portuguese, English or Spanish, published between 2012 and 2023, using the PICO strategy. 706 articles were found through the Pubmed, Embase, Web of Science, Science Direct, Scopus and Cochrane Library databases. To assess the quality of randomized clinical trials, the PEDro Scale was used. Results: The studies carried out different training sessions with varied resources, using variables such as joint position sense (JPS), postural control and balance to evaluate the proprioceptive effects of the training. The comparison groups underwent conventional rehabilitation or were healthy. In one study, joint position sense did not show a significant difference between the operated and non-operated sides, but there was a significant difference between the operated sides. Aquatic and land-based proprioceptive training compared to the group that performed conventional therapy showed proprioceptive improvement. The comparison of the balance training group with healthy people showed a partial improvement in dynamic stability indices. Cross balance exercises showed improvement in static and dynamic balance. Whole body vibration therapy did not show an improvement in joint position sense, but it did show an improvement in postural balance. Final considerations: It was possible to identify that the different types of training demonstrated positive effects with significant improvement in outcomes such as joint position sense, postural control, static balance and dynamic balance, after ACLR.

Métodos: Revisão sistemática, descrita com base no fluxograma PRISMA, incluindo ensaios clínicos randomizados em português, inglês ou espanhol, publicados entre 2012 a 2023, utilizando a estratégia PICO. Foram encontrados 706 artigos, por meio das bases de dados Pubmed, Embase, Web of Science, Science Direct, Scopus e Cochrane Library. Para avaliar a qualidade dos ensaios clínicos randomizados foi utilizado a Escala PEDro. Resultados: Os estudos realizaram diferentes treinamentos com variados recursos, utilizando variáveis de senso de posição articular (JPS), controle postural e equilíbrio para avaliar os efeitos proprioceptivos dos treinos. Os grupos comparativos realizaram reabilitação convencional ou eram saudáveis. Em um estudo o senso de posição articular não apresentou diferença significativa entre o lado operado e não operado, porém houve diferença significativa entre os lados operados. Os treinamentos proprioceptivo aquático e baseado em terra comparados com grupo que realizou a terapia convencional, apresentaram melhora proprioceptiva. A comparação do grupo de treinamento de equilíbrio com pessoas saudáveis apresentou melhora parcial nos índices de estabilidade dinâmica. Exercícios cruzado de equilíbrio mostrou melhora no equilíbrio estático e dinâmico. A terapia de vibração de corpo inteiro não apresentou melhora no senso de posição articular, porém mostrou melhora no equilíbrio postural. Considerações finais: Foi possível identificar que os diferentes tipos de treinamento demonstraram efeitos positivos com melhora significativa em desfechos como o senso de posição articular, o controle postural, o equilíbrio estático e o equilíbrio dinâmico, após a RCLA.Objetivo: Analizar los efectos del entrenamiento neuromuscular sobre la propiocepción después de la reconstrucción del ligamento cruzado anterior (ACLR). Resultados: Los estudios realizaron diferentes sesiones de entrenamiento con variados recursos, utilizando variables como el sentido de la posición articular (JPS), el control postural y el equilibrio para evaluar los efectos propioceptivos del entrenamiento. Los grupos de comparación se sometieron a rehabilitación convencional o estaban sanos. En un estudio, el sentido de la posición de las articulaciones no mostró una diferencia significativa entre los lados operados y no operados, pero sí hubo una diferencia significativa entre los lados operados. El entrenamiento propioceptivo acuático y terrestre en comparación con el grupo que realizó la terapia convencional mostró una mejora propioceptiva. La comparación del grupo de entrenamiento del equilibrio con personas sanas mostró una mejora parcial en los índices de estabilidad dinámica. Los ejercicios de equilibrio cruzado mostraron una mejora en el equilibrio estático y dinámico. La terapia de vibración de todo el cuerpo no mostró una mejora en la sensación de posición de las articulaciones, pero sí una mejora en el equilibrio postural.
