Dependence on long-term use of benzodiazepines in the treatment of anxiety in elderly patients: clonazepam versus diazepam

Resumo: | The class of benzodiazepines is among the most consumed drugs in Brazil and in the world. Developed accidentally in the 1960s, these drugs were well accepted for their satisfactory effects in controlling sleep disorders and anxiety. Due to the growing number of diagnoses of disorders related to sleep and anxiety, as well as the practice of self-medication, the abusive consumption of benzodiazepines is increasing, which consequently causes serious health problems such as tolerance, withdrawal, dependence, among others. The objective of this review is to compare the existing literature on dependence in the long-term use of benzodiazepines in the treatment of anxiety in elderly patients, highlighting Clonazepan versus Diazepam. For the bibliographic research, LILACS, PUBMED/MEDELINE and SCIELO databases were used, in which 32 articles were identified, 20 of which were used in the elaboration of the work, with the selection of articles published in national and international journals, in the period of 2013 to 2022. Results show the dependence and consequences of long-term use by elderly patients who use benzodiazepines, as well as the difficulty of management in the discontinuation of the elderly. The literature suggests the safe and effective use of benzodiazepines in elderly patients with anxiety, in order to prevent their dependence. |

Desarrollados accidentalmente en la década de 1960, estos medicamentos fueron bien aceptados por sus efectos satisfactorios en el control de los trastornos del sueño y la ansiedad. Debido al creciente número de diagnósticos de trastornos relacionados con el sueño y la ansiedad, así como la práctica de la automedicación, ha ido en aumento el consumo abusivo de benzodiazepinas, lo que en consecuencia provoca graves problemas de salud como tolerancia, abstinencia, dependencia, entre otros. El objetivo de esta revisión es comparar la literatura existente sobre dependencia en el uso a largo plazo de benzodiazepinas en el tratamiento de la ansiedad en pacientes de edad avanzada, destacando Clonazepan versus Diazepam. Para la investigación bibliográfica se utilizaron las bases de datos LILACS, PUBMED/MEDELINE y SCIELO, en las cuales se identificaron 32 artículos, de los cuales 20 fueron utilizados en la elaboración del trabajo, con selección de artículos publicados en revistas nacionales e internacionales, en el período de 2013 a 2022. Los resultados muestran la dependencia y las consecuencias del uso prolongado por parte de los ancianos que usan benzodiazepinas, así como la dificultad de manejo en la discontinuación de los ancianos. La literatura sugiere el uso seguro y eficaz de las benzodiazepinas en ancianos con ansiedad, con el fin de prevenir su dependencia.Os benzodiazepínicos estão entre as drogas mais consumidas no Brasil e no mundo. Desenvolvidas acidentalmente na década de 1960, essas drogas ganharam ampla aceitação por seus efeitos satisfatórios no controle de distúrbios do sono e ansiedade. O abuso de benzodiazepínicos está aumentando devido ao crescente diagnóstico de distúrbios do sono e relacionados à ansiedade, bem como à prática da automedicação, levando a sérios problemas de saúde, como tolerância, abstinência, dependência, dentre outras. O objetivo desta revisão é comparar a literatura existente sobre a dependência no uso prolongado dos benzodiazepínicos no tratamento da ansiedade em pacientes idosos destacando o Clonazepan versus Diazepam. Para a pesquisa bibliográfica foi utilizada as bases de dados LILACS, PUBMED/MEDELINE e SCIELO, na qual identificaram-se 32 artigos, sendo 20 utilizados na elaboração do trabalho, com a seleção de artigos publicados em periódicos nacionais e internacionais, no período de 2013 a 2022. Resultados mostram a dependência e as consequências do uso ao longo prazo por pacientes idosos que fazem o uso de benzodiazepínicos, assim como a dificuldade do manejo na descontinuação dos idosos.
