Current challenges for the eradication of hansen disease: from diagnosis to treatment

Resumo: Leprosy is a chronic, granulomatous, slowly evolving disease caused by Mycobacterium leprae, which affects the skin and peripheral nervous system, generating physical disabilities. Despite the use of multidrug therapy, the number of new cases remains constant in many countries. The possibility of global elimination of leprosy has been discussed by the WHO and by several authors, thus, the present study aims to present the challenges to be overcome in relation to its diagnosis and treatment so that this goal is achieved. For this, file searches were carried out in the SCOPUS, PUBMED, SCIELO, Google Scholar databases. Since the diagnosis of leprosy is based on a few clinical signs, many professionals have difficulty recognizing findings suggestive of the disease. One of the problems to be overcome is the absence of an exam that presents high specificity and sensitivity for all clinical forms. The development of a fast, sensitive, and non-invasive method is of great importance for the definitive diagnosis. Early diagnosis is a crucial factor for disease control and can prevent the onset of disabilities. The treatment of leprosy, its adherence and its conclusion are essential, however, the number of people who adhere to the medication has been inconsistent. In addition, the resistance of M. leprae to currently available drugs also prove to be an obstacle to be overcome. Thus, there are many obstacles that remain to be overcome so that leprosy can be eradicated in the world.

A pesar del uso de la terapia multimedicamentosa, el número de casos nuevos se mantiene constante en muchos países. La posibilidad de la eliminación global de la lepra ha sido discutida por la OMS y por varios autores, por lo que el presente trabajo tiene como objetivo presentar los desafíos a superar en relación con su diagnóstico y tratamiento para que se logre esta meta. Para ello se realizaron búsquedas de archivos en las bases de datos SCOPUS, PUBMED, SCIELO, Google Scholar. Dado que el diagnóstico de la lepra se basa en unos pocos signos clínicos, muchos profesionales tienen dificultad para reconocer hallazgos que sugieran la enfermedad. Uno de los problemas a superar es la ausencia de un examen que presente alta especificidad y sensibilidad para todas las formas clínicas. El desarrollo de un método rápido, sensible y no invasivo es de gran importancia para el diagnóstico definitivo. El diagnóstico precoz es un factor crucial para el control de la enfermedad y puede prevenir la aparición de discapacidades. El tratamiento de la lepra, su adherencia y su conclusión son fundamentales, sin embargo, el número de personas que se adhieren a la medicación ha sido inconsistente, además, la resistencia de M. leprae a los fármacos actualmente disponibles también resulta ser un obstáculo para superar. Así, son muchos los obstáculos que quedan por vencer para que la lepra pueda ser erradicada en el mundo.A hanseníase e uma doença crônica, granulomatosa, de evolução lenta causada pelo Mycobacterium leprae, que afeta pele e sistema nervoso periférico gerando incapacidades físicas. Apesar do uso da multidroga terapia, o número de novos casos ainda se mantém constante em muitos países. A possibilidade da eliminação global da hanseníase tem sido discutida pela OMS e por vários autores, desta forma, o presente trabalho tem como objetivo apresentar os desafios a ainda serem superados em relação ao seu diagnóstico e ao tratamento para que essa meta seja alcançada. Para isto, foram realizadas buscas de arquivos nas bases de dados SCOPUS, PUBMED, SCIELO, Google Acadêmico. Uma vez que o diagnóstico da hanseníase é baseado em alguns sinais clínicos, muitos profissionais têm dificuldade em reconhecer os achados sugestivos da doença. Um dos problemas serem contornados é a ausência de um exame que apresente especificidade e sensibilidade elevada para todas as formas clínicas. O desenvolvimento de um método rápido, sensível e não invasivo mostra-se de grande importância para o diagnóstico definitivo. O diagnóstico precoce é um fator crucial para o controle da doença e que pode prevenir o aparecimento de incapacidades. O tratamento da hanseníase, sua adesão e sua conclusão são essenciais, no entanto, a quantidade de pessoas que aderem a medicação tem sido inconstante. Além disso, a resistência do M. leprae aos fármacos atualmente existentes também se mostra como um obstáculo a ser superado.
