Implementation of a clinical protocol for pediatric patients with fever in a 24-hour health service - An experience report

Resumo: | Objective: The aim was to relate the experience of implementing the nursing interventional protocol for pediatrics patients with body temperature greater than 37,8 °C (100,04 °F). Methodology: This study is descriptive, exploratory, with a qualitative approach in an experience report of a medical students group and three nurses about the implementation of a clinical protocol, between the years 2019 and 2020. Results: In this protocol, it was decided to use the axillary measurement of temperature, with the fever being classified as greater than 37,8 °C (100,04 °F), contraindicating antipyretics if it was used less than 6 hours and/or drug allergies were reported. In infants older than three months, dipyrone and paracetamol were prioritized, in infants older than six months there is the possibility of ibuprofen usage. It was observed a resistance in parents and guardians in the beginning of the protocol application, meanwhile the clinical staff acceptance was immediate. The positive reflexes of teamwork and competent proactivity were observed by the academics regarding the service provided to the population. Conclusion: The implementation of protocols seeks to formalize work partnerships, acting as an instrument for standardizing the conduct and management of patients. The adoption of this measure brought benefits, providing a more organized, fast, humanized and quality service. |

Metodología: Se trata de un estudio descriptivo, exploratorio, con abordaje cualitativo del tipo relato de experiencia de un grupo de estudiantes de medicina y tres enfermeras sobre la experiencia durante la implementación de un protocolo clínico, entre los años 2019 y 2020. Resultados: En el protocolo vigente se decidió controlar la temperatura axilar, clasificándose la fiebre por encima de 37,8 ° C, antipiréticos contraindicados si había uso previo en menos de seis horas y / o reportes de alergia al medicamento. Para los niños mayores de tres meses se priorizó la administración del fármaco dipirona y paracetamol, mientras que en niños mayores de seis meses existe la posibilidad de utilizar ibuprofeno. Hubo resistencia por parte de padres y tutores al inicio de la aplicación del protocolo en el servicio, a la vez que la aceptación del personal clínico fue inmediata. Los reflejos positivos del trabajo en equipo y la proactividad competente fueron observados por los académicos con respecto al servicio brindado a la población. Conclusión: La implementación de protocolos busca formalizar las alianzas laborales, actuando como un instrumento para estandarizar la conducta y el manejo de los pacientes. La adopción de esta medida trajo beneficios, brindando un servicio más organizado, rápido, humanizado y de calidad.Objetivo: O objetivo do estudo foi relatar a experiência da implantação do protocolo de intervenção da enfermagem para pacientes pediátricos com temperatura corporal maior que 37,8°C. Metodologia: Trata-se de um estudo descritivo, exploratório, com abordagem qualitativa do tipo relato de experiência de um grupo de discentes de medicina e três enfermeiros sobre a vivência durante a implantação de um protocolo clínico, entre os anos de 2019 e 2020. Resultados: No protocolo em vigor decidiu-se por aferir a temperatura axilar, sendo a febre classificada acima de 37,8°C, contraindicado antitérmicos se houver uso anterior em período inferior a seis horas e/ou relatos de alergia ao medicamento. Para crianças acima de três meses de idade, priorizou-se a administração dos fármacos dipirona e paracetamol, já em crianças maiores de seis meses há a possibilidade do uso de ibuprofeno. Observou-se uma resistência dos pais e responsáveis no início da aplicação do protocolo no serviço, enquanto a aceitação do corpo clínico foi imediata. Os reflexos positivos do trabalho em equipe e da proatividade competente foram observados pelas acadêmicas frente ao atendimento prestado à população. Conclusão: A implantação de protocolos busca formalizar parcerias de trabalho, atuando como instrumento de padronização de condutas e manejo dos pacientes.
