Intrauterine growth restriction: etiology associated with maternal and placental causes

Resumo: Intrauterine Growth Restriction (IUU) is an important complication of pregnancy due to high rates of perinatal morbidity and mortality. This complication is defined as a fetus that cannot reach its growth percentage, caused by maternal and placental factors. Thus, this study aimed to evaluate maternal and placental determinants, as well as to specify diagnostic methods and possible outcomes in the face of IUD. A literature review was conducted in the PubMed, SCIELO and VHL databases, having as inclusion criteria: articles published in the period from 2010 to 2022 in the language in Portuguese and English. 16 publications eligible for inclusion in this review were found. In view of the analysis of the literature, it was noted that there are several determinants of IUR, such as: age, nutritional status, hypertension, chronic kidney disease, diabetes, illicit drug use, tobacco, ethnicity, anemia, abnormal implantation of the placenta, single umbilical artery, placenta previa, uterine abnormalities, circumvalized placenta, placental infarctions and others. To identify the fetus with this delay, the measurement of uterine height (AU), obstetric ultrasound, fetal weight estimation (PEE) and umbilical artery Doppler are used. As for the possible results in this pathology, it can lead to probable complications for the newborn and in childhood, consistent with changes in health condition. Thus, it is concluded that there are multiple factors related to IUEd and that the identification of the main changes involved by diagnostic methods is of fundamental importance to avoid problems in the health of the fetus and pregnant women.

Esta complicación se define como un feto que no puede alcanzar su porcentaje de crecimiento, causado por factores maternos y placentarios. Por lo tanto, este estudio tuvo como objetivo evaluar los determinantes maternos y placentarios, así como especificar los métodos de diagnóstico y los posibles resultados frente al DIU. Se realizó una revisión bibliográfica en las bases de datos PubMed, SCIELO y BVS, teniendo como criterios de inclusión: artículos publicados en el período de 2010 a 2022 en el idioma en portugués e inglés. Se encontraron 16 publicaciones elegibles para su inclusión en esta revisión. En vista del análisis de la literatura, se observó que existen varios determinantes del DIUr, tales como: edad, estado nutricional, hipertensión, enfermedad renal crónica, diabetes, consumo de drogas ilícitas, tabaco, etnia, anemia, implantación anormal de la placenta, arteria umbilical única, placenta previa, anomalías uterinas, placenta circunvalizada, infartos placentarios y otros. Para identificar al feto con este retraso, se utiliza la medición de la altura uterina (AU), la ecografía obstétrica, la estimación del peso fetal (PEE) y el Doppler de la arteria umbilical. En cuanto a los posibles resultados en esta patología, puede conducir a probables complicaciones para el recién nacido y en la infancia, consistentes con cambios en el estado de salud. Así, se concluye que existen múltiples factores relacionados con el DIU y que la identificación de los principales cambios que implican los métodos diagnósticos es de fundamental importancia para evitar problemas en la salud del feto y de las mujeres embarazadas.A Restrição do Crescimento Intrauterino (RCIU) é uma complicação importante da gravidez, devido a taxas elevadas de morbidade e mortalidade perinatal. Esta complicação é definida como um feto que não consegue atingir seu percentual de crescimento, ocasionado por fatores maternos e placentários. Desta forma, este estudo teve como objetivo avaliar os determinantes maternos e placentários, bem como especificar os métodos diagnósticos e os possíveis desfechos diante da RCIU. Foi realizada uma revisão de literatura nas bases de dados PubMed, SCIELO e BVS, tendo como critérios de inclusão: artigos publicados no período de 2010 a 2022 no idioma em português e inglês. Sendo encontradas 16 publicações elegíveis para inclusão desta revisão. Diante da análise da literatura notou-se que são vários os determinantes causadores da RCIU, como: a idade, estado nutricional, hipertensão, doença renal crônica, diabetes, uso de drogas ilícitas, tabaco, etnia, anemia, implantação anormal da placenta, artéria umbilical única, placenta prévia, anormalidades uterinas, placenta circunvalada, infartos da placenta e entre outros. Para identificação do feto com este retardo é utilizado a medida da altura uterina (AU), a ultrassonografia obstétrica, a estimativa do peso fetal (PEE) e o Doppler da artéria umbilical. Quanto aos possíveis resultados diante desta patologia pode acarretar prováveis complicações ao recém-nascido e na infância, condizentes a alterações na condição de saúde.
