Sugamadex in the reversal of anaphylaxis due to rocuronium in a pediatric patient: Case report

Resumo: Neuromuscular blockers, notably rocuronium, are a prominent cause of anaphylactic reactions in the perioperative period. Rocuronium, known for its rapid action and intermediate duration, has gained prominence due to the benefits of Sugammadex, its reversal agent, which provides agile and predictable reversal of neuromuscular blockade. We report the case of a 6-year-old patient, weighing 30 kg, who underwent video-assisted thoracoscopic surgery (VATS), who presented an adverse reaction after the administration of rocuronium, manifesting chest flushing, tachycardia, severe hypotension and generalized petechiae. Despite attempts at treatment with adrenaline, there was no improvement in hemodynamics. After administration of Sugammadex, a positive response was observed, with an increase in heart rate and blood pressure, allowing the patient to be transferred to the Pediatric Intensive Care Unit. In this sense, this case report aimed to describe the treatment of an episode of anaphylaxis induced by rocuronium and treated with the administration of sugammadex.

El rocuronio, conocido por su rápida acción y duración intermedia, ha ganado protagonismo debido a los beneficios del Sugammadex, su agente reversor, que proporciona una reversión ágil y predecible del bloqueo neuromuscular. Se presenta el caso de un paciente de 6 años de edad, con peso de 30 kg, sometido a cirugía toracoscópica videoasistida (VATS), quien presentó una reacción adversa luego de la administración de rocuronio, manifestando enrojecimiento torácico, taquicardia, hipotensión severa y petequias generalizadas. . A pesar de los intentos de tratamiento con adrenalina, no hubo mejoría en la hemodinámica. Luego de la administración de Sugammadex se observó respuesta positiva, con aumento de la frecuencia cardíaca y presión arterial, permitiendo el traslado del paciente a la Unidad de Cuidados Intensivos Pediátricos. En este sentido, el presente reporte de caso tuvo como objetivo describir el tratamiento de un episodio de anafilaxia inducido por rocuronio y tratado con la administración de sugammadex.Bloqueadores neuromusculares, notadamente o rocurônio, são uma causa proeminente de reações anafiláticas no período perioperatório. O rocurônio, conhecido por sua ação rápida e de duração intermediária, tem ganhado destaque devido aos benefícios do Sugamadex, seu agente reversor, que proporciona reversão ágil e previsível de bloqueio neuromuscular. Relatamos o caso de um paciente de 6 anos, 30 kg, submetido a vídeotoracoscopia assistida (VATS), que apresentou uma reação adversa após a administração de rocurônio, manifestando rubor torácico, taquicardia, hipotensão grave e petéquias generalizadas. Apesar das tentativas de tratamento com adrenalina, não houve melhora na hemodinâmica. Após a administração de Sugamadex, observou-se uma resposta positiva, com aumento da frequência cardíaca e pressão arterial, permitindo a transferência do paciente para a Unidade de Terapia Intensiva Pediátrica.
