Biochemical Changes in Methotrexate Users in Combination with Leflunomide for Rheumatoid Arthritis

Resumo: Objective. To analyze the biochemical changes of patients with Teresina-PI CEAF using methotrexate monotherapy and in combination with leflunomide for the treatment of rheumatoid arthritis. Methods. The research was characterized in a cross-sectional, descriptive study, with a quantitative and qualitative approach where data collection was carried out by searching the National System of Management of Pharmaceutical Services - HORUS and through consultations of the documentary records of users assisted by CEAF based on therapeutic treatment, drug interference in laboratory parameters, treatment time, sex and changes in therapeutic treatment. All statistical analyses were performed using the Statistical Package for the Social Science (SPSS) software, to obtain these analyses we used the Mann-Whitney U test for independent variables with a 95% confidence level. For a significant result, the p-value needs to be lower than the significance level α=0.05. Results. 98 patients selected for the study, 90,81 % were female, 9.18 % male, in relation to drug treatment there were similarities with the characteristic findings, 92.85 % of these patients were using MTX + LEF with association and 7.14 % were using monotherapy with MTX. The patients in both treatment presented alterations in laboratory parameters at the two time intervals, with a greater change in liver enzymes AST/ALT in the shortest period of time, creatinine was the test that showed the least alteration, with a slight change in patients on associated treatment in the period of time greater than one year. Conclusion. In view of the analysis of the results, it was found that there was no statistically significant difference between the laboratory parameters analyzed: liver enzymes AST / ALT, hemoglobin and creatinine of patients treated with MTX in isolated therapy and with MTX associated with LEF.

Analizar los cambios bioquímicos de los pacientes del CEAF de Teresina - PI en monoterapia con metotrexato y en combinación con leflunomida para el tratamiento de la artritis reumatoide. Metodología. La investigación se caracterizó por un estudio descriptivo, transversal, con abordaje cuantitativo y cualitativo donde la recolección de datos se realizó mediante la búsqueda en el Sistema Nacional de Gestión de Asistencia Farmacéutica - HÓRUS y la consulta de registros documentales de usuarios asistidos por CEAF. basado en tratamiento terapéutico, interferencias de fármacos en parámetros de laboratorio, tiempo de tratamiento, sexo y cambios en el tratamiento terapéutico. Todos los análisis estadísticos se realizaron utilizando el software Statistical Package for the Social Science (SPSS), para obtener estos análisis utilizamos la prueba U de Mann-Whitney para variables independientes con un nivel de confianza del 95%. Para un resultado significativo, el valor p debe ser menor que el nivel de significancia α = 0.05. Resultados. 98 pacientes seleccionados para el estudio, el 90,81 % eran mujeres, el 9,18 % eran hombres, en relación al tratamiento farmacológico había similitudes con los hallazgos característicos, el 92,85 % de estos pacientes usaban MTX + LEF y El 7,14 % utilizó monoterapia con MTX. Los pacientes en ambos tratamientos mostraron cambios en los parámetros de laboratorio en los dos intervalos de tiempo, notando un mayor cambio en las enzimas hepáticas AST / ALT en el menor tiempo, la creatinina fue la prueba que mostró el menor cambio, presentando un ligero cambio en pacientes en tratamiento asociado durante un período de tiempo superior a un año. Conclusión. A la vista del análisis de los resultados, se encontró que no existía diferencia estadísticamente significativa entre los parámetros de laboratorio analizados: enzimas hepáticas AST / ALT, hemoglobina y creatinina de pacientes tratados con MTX en terapia aislada y con MTX asociado a LEF.Objetivo.  Analisar as alterações bioquímicas dos pacientes da CEAF de Teresina – PI em uso da monoterapia com metotrexato e em combinação com leflunomida para o tratamento da artrite reumatóide. Metodologia. A pesquisa caracterizou-se em um estudo transversal, descritivo, com a abordagem quantitativa e qualitativa onde as coletas dos dados foram realizadas mediante a busca no Sistema Nacional de Gestão da Assistência Farmacêutica - HÓRUS e através de consultas dos registros documentais dos usuários assistidos pelo CEAF tendo como base o tratamento terapêutico, interferências medicamentosas nos parâmetros laboratoriais, tempo de tratamento, sexo e alterações no tratamento terapêutico. Todas as análises estatísticas foram realizadas usando o software Statistical Package for the Social Science (SPSS), para obter essas análises utilizamos o teste de U de Mann- Whitney para variáveis independentes com nível de 95% de confiança. Para um resultado significativo, o p-valor precisa ser menor que o nível de significância α=0,05. Resultados. 98 pacientes selecionados para o estudo, 90,81 % eram do sexo feminino, 9,18 % do sexo masculino, em relação ao tratamento medicamentoso houve semelhanças com os achados característicos, 92,85 % desses pacientes estavam fazendo uso associado MTX + LEF e 7,14 % faziam uso da monoterapia com o MTX. Os pacientes em ambos o tratamento apresentaram alterações nos parâmetros laboratoriais nos dois intervalos de tempo, notando-se uma maior alteração nas enzimas hepáticas AST/ALT no menor período de tempo, a creatinina foi o exame que menos mostrou alteração, apresentando uma discreta alteração nos pacientes em tratamento associado no período de tempo maior que um ano. Conclusão.
