Frequency and specificity of erythrocyte alloimmunization in patients at a reference hospital in Alagoas, Brazil

Resumo: | Objective: To determine the frequency and specificity of erythrocyte alloimmunization in patients treated by hemotherapy at the University Hospital Professor Alberto Antunes - UFAL, Maceió-Alagoas, in the period 2013-2018. Methods: Retrospective analysis all patient records with request for red blood cell concentrate (RBC) with a positive erythrocyte antibody test. Results: The incidence of erythrocyte alloimmunization was higher in women n=144 (80.45%) than in men n=35 (19.55%). We detected 13 antibodies, the most frequent specificities identified were anti-D (35.20%), anti-M (12.30%) and anti-E (7.82%). The combination of antibodies occurred in twenty-six cases with prevalence for combinations of antibodies of the Rh system, anti-D + anti-C (23.08%). The highest incidence of alloantibodies was observed in pregnant women. Conclusion: The population studied had high levels of alloimmunization through the Rh System, greater specificity of alloimmunization in women through anti-D and higher rate of alloimmunization among men over the age of 40. Further studies are necessary for a better understanding of red blood cell alloimmunization for population of Alagoas. |

Método: Análisis retrospectivo de todas las historias clínicas de los pacientes que solicitaron concentrado de glóbulos rojos (RBC) con prueba de anticuerpos contra eritrocitos positiva. Resultados: La incidencia de aloinmunización eritrocitaria fue mayor en mujeres n=144 (80,45%) que en hombres n=35 (19,55%). Detectamos 13 anticuerpos, siendo las especificidades más frecuentemente identificadas anti-D (35,20%), anti-M (12,30%) y anti-E (7,82%). La combinación de anticuerpos ocurrió en veintiséis casos con predominio para combinaciones de anticuerpos del sistema Rh, anti-D + anti-C (23,08%). La mayor incidencia de aloanticuerpos se observó en mujeres embarazadas. Conclusión: La población estudiada presentó altos niveles de aloinmunización por el Sistema Rh, mayor especificidad de aloinmunización en mujeres por anti-D y mayor tasa de aloinmunización en hombres mayores de 40 años. Son necesarios nuevos estudios para una mejor comprensión de la aloinmunización eritrocitaria en la población de Alagoas.Objetivo: Determinar a frequência e especificidade da aloimunização eritrocitária em pacientes tratados por hemoterapia no Hospital Universitário Professor Alberto Antunes - UFAL, Maceió-Alagoas, no período de 2013-2018. Resultados: A incidência de aloimunização eritrocitária foi maior em mulheres n = 144 (80,45%) do que em homens n = 35 (19,55%). Detectamos 13 anticorpos, sendo as especificidades identificadas com maior frequência anti-D (35,20%), anti-M (12,30%) e anti-E (7,82%). A combinação de anticorpos ocorreu em vinte e seis casos com prevalência para combinações de anticorpos do sistema Rh, anti-D + anti-C (23,08%). A maior incidência de aloanticorpos foi observada em mulheres grávidas. Conclusão: A população estudada apresentou altos níveis de aloimunização pelo Sistema Rh, maior especificidade de aloimunização em mulheres por anti-D e maior índice de aloimunização em homens acima de 40 anos.
