Efficacy of guided endodontics in treating teeth with radicular calcification: integrative review

Resumo: Pulp calcification is commonly found in anterior teeth and in dental elements of older people, being considered a pathological response given by the pulp to injuries suffered by these teeth. Calcifications are associated with trauma, caries and age of the individual, where the root canal is totally or partially obliterated by deposition of mineral tissue. The objective of this work was to carry out an integrative review of the current literature on Guided Endodontics (EG) in teeth with root calcification, highlighting its performance technique, indications, limitations, advantages and disadvantages. An integrative literature review was carried out by searching scientific articles in the electronic database PubMed using the descriptors: guided endodontics, access, root calcification, indexed in the period from 2014 to 2019 that dealt with case reports, in vitro study and ex vivo. After the eligibility criteria, 11 articles published in English were analyzed. Studies have shown that the technique brings more safety to endodontic intervention in teeth with calcification, decreasing the chances of iatrogenesis, such as perforations. The guided access for the endodontic treatment of teeth with root calcification demonstrated high reliability, allowing a more predictable and quick root canal location, in addition to providing a conservative access, with relatively minor dentinal damage, optimizing the treatment.

Las calcificaciones están asociadas con traumatismos, caries y edad del individuo, donde el conducto radicular se borra total o parcialmente por la deposición de tejido mineral. El objetivo de este trabajo fue llevar a cabo una revisión integradora de la literatura actual sobre Endodoncia Guiada (EG) en dientes con calcificación de la raíz, destacando su técnica de rendimiento, indicaciones, limitaciones, ventajas y desventajas. Se realizó una revisión bibliográfica integradora mediante la búsqueda de artículos científicos en la base de datos electrónica PubMed utilizando los descriptores: endodoncia guiada, acceso, calcificación de la raíz, indexada en el período de 2014 a 2019 que trató los informes de casos, estudio in vitro y ex vivo. Después de los criterios de elegibilidad, se analizaron 11 artículos publicados en inglés. Los estudios han demostrado que la técnica aporta más seguridad a la intervención endodóntica en dientes con calcificación, disminuyendo las posibilidades de iatrogénesis, como las perforaciones. El acceso guiado para el tratamiento endodóntico de los dientes con calcificación de la raíz demostró una alta confiabilidad, lo que permitió una ubicación del canal radicular más predecible y rápida, además de proporcionar un acceso conservador, con daño dentinal relativamente menor, optimizando el tratamiento.A calcificação pulpar é comumente encontrada em dentes anteriores e em elementos dentários de pessoas mais velhas, sendo considerada uma resposta patológica dada pela polpa às injúrias sofridas por esses dentes. As calcificações são associadas aos traumatismos, cárie e idade do indivíduo, onde o canal radicular é obliterado totalmente ou parcialmente por deposição de tecido mineral. O objetivo desse trabalho foi realizar uma revisar integrativa da literatura atual sobre a Endodontia Guiada (EG) em dentes com calcificação radicular, destacando sua técnica de realização, indicações, limitações, vantagens e desvantagens. Uma revisão integrativa da literatura foi realizada por meio de uma busca de artigos científicos na base de dados eletrônica PubMed utilizando os descritores: guided endodontics, access, root calcification, indexados no período de 2014 a 2019 que tratavam de relato de caso, estudo in vitro e ex vivo. Após os critérios de elegibilidade foram analisados 11 artigos integralmente publicados em língua inglesa. Os estudos mostraram que a técnica traz mais segurança para a intervenção endodôntica em dentes com calcificação, diminuindo as chances de iatrogenia, como as perfurações.
