Physical therapy effectiveness in patients with urinary incontinence after radical prostatectomy: A literature review

Resumo: | The incidence of prostate cancer (PCa) has increased worldwide and nationally. The scenario of the growing number of cases has resulted in an expansion of the means of early diagnosis used to detect the pathology. Early detection is responsible for reducing the death rates caused by the disease. The gold standard treatment for PCa is Radical Prostatectomy (RP), which is the surgical approach for the complete removal of the prostate, and among its side effects is Urinary Incontinence (UI). This dysfunction occurs mainly due to the removal of part of the urethral sphincter as a consequence of the procedure. The objective of this work was to analyze by means of a bibliographic review the efficiency of Physical Therapy in patients with Urinary Incontinence after Radical Prostatectomy. The method used was bibliographic research in national and international articles in databases such as Pubmed, Scielo, BIREME and LILACS. 67 works were found, from which 9 were selected. The selected articles presented evaluation of the effect of physical therapy treatment in patients with UI after RP. It was possible to find evidence of UI reduction regardless of the technique used. However, even with evidence of acquisition of continence after RP, a standardization of methods and protocols used was not found. Therefore, it is suggested that more studies be conducted in the area. |

El escenario del creciente número de casos se ha traducido en una ampliación de los medios de diagnóstico precoz utilizados para detectar la patología. La detección precoz es responsable de la reducción de las tasas de mortalidad causadas por la enfermedad. El tratamiento de referencia para el CaP es la prostatectomía radical (PR), que es el abordaje quirúrgico para la extirpación completa de la próstata, y entre sus efectos secundarios se encuentra la incontinencia urinaria (IU). Esta disfunción se debe principalmente a la eliminación de parte del esfínter uretral como consecuencia del procedimiento. El objetivo del trabajo fue analizar mediante revisión bibliográfica la efectividad de la Fisioterapia en pacientes con Incontinencia Urinaria tras Prostatectomía Radical. El método utilizado fue la pesquisa bibliográfica en artículos nacionales e internacionales en bases de datos como Pubmed, Scielo, BIREME y LILACS. Fueron encontrados 67 trabajos, de los cuales fueron seleccionados 9. Los artículos seleccionados presentaron evaluación del efecto del tratamiento fisioterápico en pacientes con IU después de la PR. Fue posible encontrar pruebas sobre la reducción de la IU independientemente de la técnica utilizada. Sin embargo, incluso con pruebas de adquisición de continencia tras la PR, no se encontró una estandarización de los métodos y protocolos utilizados. Por lo tanto, se sugiere que se realicen más estudios en la zona.A incidência do câncer de próstata (CaP) tem aumentado no mundo e nacionalmente. O cenário do crescente número de casos tem resultado em uma expansão dos meios de diagnósticos precoce utilizados para a detecção da patologia. A detecção precoce é responsável pela redução dos índices de óbito causados pela doença. O CaP tem como tratamento padrão ouro a Prostatectomia Radical (PR) que é a abordagem cirúrgica para a remoção completa da próstata e entre seus efeitos colaterais está a Incontinência Urinária (IU). Essa disfunção ocorre principalmente devido à remoção de parte do esfíncter uretral como consequência do procedimento. O objetivo do trabalho foi analisar por meio de revisão bibliográfica a eficácia da Fisioterapia em pacientes com Incontinência Urinária pós Prostatectomia Radical. O método utilizado foi pesquisa bibliográfica em artigos nacionais e internacionais em bases de dados como Pubmed, Scielo, BIREME e LILACS. Foram recuperados 67 trabalhos e desses, 9 selecionados. Os artigos selecionados apresentaram avalição do efeito do tratamento fisioterapêutico em pacientes com IU após a PR. Foi possível encontrar evidências sobre a redução da IU independentemente da técnica utilizada. No entanto, mesmo com evidências de aquisição da continência após a cirurgia de PR, não foi encontrada uma padronização dos métodos e dos protocolos utilizados.
