Neck circumference as a predictor of cardiovascular risk in chronic kidney disease patients on hemodialysis

Resumo: | The present study aimed to evaluate neck circumference as a predictive parameter of cardiovascular risk in renal patients undergoing hemodialysis. This is a cross-sectional study with 103 patients with chronic kidney disease undergoing hemodialysis, evaluated from July 2018 to September 2019, in two hemodialysis clinics located in the metropolitan region of Recife-PE. Cardiovascular risk was assessed from neck circumference using the Neck Circumference ≥ 37 cm for men and ≥ 34 cm for women cut-off points, as described by BenNoun, Sohar and Laor. Sociodemographic and lifestyle characteristics, as well as traditional anthropometric parameters and indices, were evaluated. In addition, biochemical parameters such as lipid profile, blood glucose, vitamin D, among others, were evaluated. To assess the association between the variables and cardiovascular risk, the chi-square with Yates correction and the McNemar test were used. To verify the correlations, Pearson's correlation was used. Variables with a value of p≤10 were considered borderline significance and those with p<0.05 were considered statistically significant. Neck circumference is associated with HDL (p<0.001), LDL (p<0.001) and blood glucose (p<0.001) and is correlated with waist circumference (p<0.001), conicity index (p<0.001), waist-to-height ratio (p<0.001), and waist-to-hip ratio (p<0.001), body mass index (p<0.001), arm circumference (p=0.04), as well as serum levels of vitamin D (p<0.001). It is concluded that neck circumference is a good predictor of cardiovascular risk in this population. |

Se trata de un estudio transversal con 103 pacientes con enfermedad renal crónica en hemodiálisis, evaluados entre el período de julio de 2018 y septiembre de 2019, en dos clínicas de hemodiálisis ubicadas en la región metropolitana de Recife-PE. El riesgo cardiovascular se evaluó a partir de la circunferencia del cuello utilizando los puntos de corte de Circunferencia del cuello ≥ 37 cm para hombres y ≥ 34 cm para mujeres, según lo descrito por BenNoun, Sohar y Laor. Se evaluaron características sociodemográficas y de estilo de vida, así como parámetros e índices antropométricos tradicionales. Además, se evaluaron parámetros bioquímicos como perfil lipídico, glucemia, vitamina D, entre otros. Para evaluar la asociación entre las variables y el riesgo cardiovascular se utilizó el chi-cuadrado con corrección de Yates y el test de McNemar. Para verificar las correlaciones se utilizó la correlación de Pearson. Las variables con un valor de p≤10 fueron consideradas de significancia limítrofe y aquellas con p<0,05 fueron consideradas estadísticamente significativas. La circunferencia del cuello se asocia con HDL (p<0,001), LDL (p<0,001) y glucosa en sangre (p<0,001), y se correlaciona con la circunferencia de la cintura (p<0,001), índice de conicidad (p<0,001), cintura-a índice de estatura (p<0,001), índice cintura-cadera (p<0,001), índice de masa corporal (p<0,001), circunferencia del brazo (p=0,04), así como niveles séricos de vitamina D (p< 0,001). O risco cardiovascular foi avaliado a partir da circunferência do pescoço utilizando os pontos de corte de Circunferência do Pescoço ≥ 37 cm para homens e ≥34 cm para mulheres, conforme descrito por BenNoun, Sohar e Laor. Foram avaliadas características sociodemográficas e de estilo de vida, bem como parâmetros e índices antropométricos tradicionais. Além disso, foram avaliados parâmetros bioquímicos, como perfil lipídico, glicemia, vitamina D, entre outros. Para avaliar a associação entre as variáveis e o risco cardiovascular foi utilizado o Qui-quadrado com correção de Yates e o teste de McNemar. Para verificar as correlações foi utilizado a correlação de Pearson. As variáveis com um valor de p≤10 foram consideradas com significância limítrofe e as com p <0,05, foram considerados estatisticamente significantes. A Circunferência do Pescoço está associada ao HDL (p<0,001), LDL (p<0,001) e Glicemia (p<0,001), e está correlacionada com a circunferência da cintura (p<0,001), índice de conicidade (p<0,001), razão cintura-estatura (p<0,001), e razão cintura- quadril (p<0,001), índice de massa corporal (p<0,001), circunferência do braço (p=0,04), bem como como os níveis séricos de vitamina D (p<0,001).
