Impact of anti-Tumour Necrosis Factor (Anti-TNF) Biological Therapy on Insulin Sensitivity in Patients with Crohn's Disease: A Literature Review

Resumo: Introduction: Crohn's disease is an inflammatory bowel disease that affects any part of the gastrointestinal tract, from the mouth to the anus, resulting in an exacerbated inflammatory process. Medication-based approaches have been effective in symptom control, and among them, biological therapy serves as an ally in maintaining patient remission. Objective: To report the action of anti-TNF biological treatment on insulin profiles in patients with Crohn's disease, considering that insulin sensitivity is impaired by the high expression of TNF-alpha in the disease. Methods: A narrative review was conducted, collecting articles from the last 10 years to seek current scientific evidence on the topic. Six steps were followed for this review, utilizing the following databases: Google Scholar, LILACS, National Library of Medicine (PubMed), and SciELO. The final number of articles was determined through a thematic similarity analysis, considering the research objective. Results: After the searches, 125 articles were identified, but only 55 articles were selected for analysis and discussion. The literature review demonstrates that insulin resistance may be higher in obese patients at risk of developing type 2 diabetes. Conclusion: In summary, it is shown that there is a relationship between metabolic diseases and inflammatory bowel disease, and anti-TNF biological therapy may be effective in both situations.

El enfoque farmacológico ha sido efectivo en el control de los síntomas, y entre las opciones disponibles, la terapia biológica se presenta como aliada para mantener la remisión del paciente. Objetivo: Informar sobre la acción del tratamiento biológico anti-TNF en el perfil de la insulina en pacientes con enfermedad de Crohn, ya que la sensibilidad a la insulina se ve perjudicada por la alta expresión de TNF-alfa en la enfermedad. Métodos: A través de una revisión narrativa, se recopilaron artículos de los últimos 10 años con el propósito de buscar evidencia científica actual sobre el tema. Para realizar esta revisión, se siguieron seis etapas y se utilizaron las siguientes bases de datos: Google Académico, LILACS, National Library of Medicine (PubMed) y SciELO. La determinación del número final de artículos se realizó mediante análisis de similitud con el tema, considerando el objetivo de la investigación. Resultados: Tras las búsquedas, se identificaron 125 artículos, pero solo se seleccionaron 55 para análisis y discusión. La revisión de literatura muestra que el índice de resistencia a la insulina puede ser mayor en pacientes obesos con riesgo de desarrollar diabetes tipo 2. Conclusión: En resumen, se demuestra que hay una relación entre las enfermedades metabólicas y la enfermedad inflamatoria del intestino y que la terapia biológica anti-TNF puede ser efectiva en ambas situaciones.Introdução: A doença de Crohn é uma doença inflamatória intestinal que acomete qualquer parte do trato gastrointestinal, da boca ao ânus, e que resulta em um processo inflamatório exacerbado. A abordagem medicamentosa tem sido eficaz no controle dos sintomas, e dentre as existentes, a terapia biológica entra como aliada para manter a remissão do paciente. Objetivo: Relatar a ação do tratamento biológico anti-TNF no perfil da insulina em pacientes com Doença de Crohn, visto que a sensibilidade à insulina é prejudicada pela alta expressão de TNF-alfa na doença. Métodos: Através de uma revisão narrativa foram selecionados artigos dos últimos 10 anos, com o propósito de buscar evidências científicas atuais sobre o tema. Para realizar essa revisão, foram seguidas seis etapas e utilizadas as seguintes bases de dados: Google Acadêmico; LILACS; National Library of Medicine (PubMed) e SciELO. A determinação do número final de artigos foi realizada através da análise de similaridade com o tema, levando em consideração o objetivo da pesquisa.
