Factors associated with neonatal mortality of very low birth weight preterm infants in an Intensive Care Unit

Resumo: Objective: to analyze the factors associated with mortality up to 28 days in premature infants with very low birth weight (<1500 g). Methods:Retrospective cohort study with a quantitative nature, and data from medical records of children born from July 31, 2015 to August 1, 2016 were collected, based on the delivery records in the delivery room and summary of hospitalization in Neonatal ICU of a public maternity hospital in Maranhão. The variables studied were: maternal characteristics, characteristics of the newborn at birth and neonatal evolution, associated with mortality up to 28 days. The variables associated with death were determined by logistic regression, estimating the odds ratios with 95% CI. Results: Of the 209 newborns studied, 87 (41.6%) died. They showed a significant association with death: gestational age less than 28 weeks (OR = 4.99; CI = 95%: 2.02-12.32), birth weight less than 1000 g (OR = 4.34; CI = 95%: 1.87-10.04), APGAR in the 5 'less than 7 (OR = 4.44; CI = 95%: 1.46-13.48), use of surfactant (OR = 3.72 ; 95% CI: 1.64-8.43) and who had pulmonary hemorrhage (OR = 19.17; CI = 95%: 4.46-82.42). Maternal characteristics did not remain in the final model, which suggests that the death that occurred in the neonatal period is influenced by characteristics of the newborn and by care factors. Conclusion: The causes of death in the neonatal period were associated with lower gestational age, lower birth weight, low vitality at birth, as well as the use of surfactant and the presence of pulmonary hemorrhage. There is a need to identify and establish practices to improve care for pregnancy, childbirth, and the newborn.

Métodos:Estudio de cohorte retrospectivo con carácter cuantitativo, y recopilación de datos de historia clínica de niños nacidos entre el 31 de julio de 2015 y el 1 de agosto de 2016, a partir de los relatos de nacimiento en la sala de partos y resumen de hospitalización en UCI Neonatal de una maternidad públicaen Maranhão. Las variables estudiadas fueron: características maternas, características del recien nacido al nacer y evolución neonatal, asociadas a la mortalidad hasta los 28 días. Las variables asociadas a la muerte se determinaron mediante regresión logística, estimando los odds ratios con IC 95%. Resultados: De los 209 recién nacidos estudiados, 87 (41,6%) fallecieron. Mostraron una asociación significativa con la muerte: edad gestacional menor de 28 semanas (OR = 4,99; IC = 95%: 2,02-12,32), peso al nacer menor de 1000 g (OR = 4,34; IC = 95%: 1,87-10,04), APGAR en los 5 'menos de 7 (OR = 4,44; IC = 95%: 1,46-13,48), uso de surfactante (OR = 3,72; IC 95%: 1,64-8,43) y que tuvieran hemorragia pulmonar (OR = 19,17; IC = 95%: 4,46-82,42). Las características maternas no permanecieron en el modelo final. Conclusión: Las causas de muerte en el período neonatal se asociaron con menor edad gestacional, menor peso al nacer, baja vitalidad al nacer, así como el uso de surfactante y la presencia de hemorragia pulmonar.Objetivo: analisar os fatores associados à mortalidade até 28 dias em prematuros com muito baixo peso ao nascimento (<1500 g). Métodos: Estudo de coorte com desenho retrospectivo, de natureza quantitativa, e coletados dados de prontuários de crianças nascidas no período de 31 de julho de 2015 a 1 de agosto de 2016, tendo como base as fichas de atendimento em sala de parto e sumário de internação em UTI Neonatal de uma maternidade pública do Maranhão. As variáveis estudadas foram: características maternas, características do recém-nascido ao nascimento e evolução neonatal, associadas à mortalidade até 28 dias. As variáveis associadas ao óbito foram determinadas por regressão logística, estimando o odds ratios com IC de 95%. Resultados: Dos 209 recém-nascidos estudados, 87 (41,6%) tiveram óbito. Apresentaram associação significativa com o óbito: idade gestacional menor do que 28 semanas (OR=4,99; IC=95%: 2,02-12,32), peso ao nascer menor de 1000 g (OR=4,34; IC=95%: 1,87-10,04), APGAR no 5’ menor que 7 (OR=4,44; IC=95%: 1,46-13,48), uso de surfactante (OR=3,72; IC95%: 1,64-8,43) e que apresentaram hemorragia pulmonar (OR=19,17; IC=95%: 4,46-82,42). Características maternas não permaneceram no modelo final.
