Use of multiparametric magnetic resonance in the active surveillance of prostate cancer: An integrative review

Resumo: Introduction: Prostate cancer is the second most common malignancy in males, with an increasing incidence due to improvements in diagnostic methods, such as multiparametric magnetic resonance imaging (MRI). Given this, new therapeutic modalities, such as active surveillance (AV), which is indicated for low-risk prostate cancer, are gaining ground. Objective: To investigate the use of mpMRI for monitoring patients undergoing active surveillance as a treatment for prostate cancer. Methods: This is an integrative review carried out using the SciELO, PubMed ®, VHL and LILACS databases. In total, 389 studies were found, after applying the inclusion and exclusion criteria, 17 articles remained chosen. Results: 5,206 patients with prostate cancer were analyzed in 15 studies, as there was no sample in 2 selected articles. Some studies assessed that mpMRI was not sufficient for monitoring VA patients with prostate cancer, with an indication for supplementation with biopsy even when there was no indication of disease progression on mpMRI. Despite this, most of the selected studies concluded that mpMRI had a negative predictive value for prostate cancer progression and could be used for follow-up in VA patients, with improvement if related to other variables such as prostate-specific antigen levels. Conclusion: mpMRI is gaining importance in the evaluation of patients with prostate cancer, including the indication and follow-up of patients in VA. Despite some limitations, it can be used in conjunction with other components to assess disease progression.

Ante esto, están ganando terreno nuevas modalidades terapéuticas, como la vigilancia activa (AV), que está indicada en el cáncer de próstata de bajo riesgo. Objetivo: Investigar el uso de mpMRI para el seguimiento de pacientes sometidos a vigilancia activa como tratamiento para el cáncer de próstata. Métodos: Se trata de una revisión integradora realizada utilizando las bases de datos SciELO, PubMed ®, BVS y LILACS. En total se encontraron 389 estudios, luego de aplicar los criterios de inclusión y exclusión quedaron elegidos 17 artículos. Resultados: Se analizaron 5.206 pacientes con cáncer de próstata en 15 estudios, al no existir muestra en 2 artículos seleccionados. Algunos estudios evaluaron que la mpMRI no era suficiente para monitorear a los pacientes VA con cáncer de próstata, con indicación de suplementación con biopsia incluso cuando no había indicación de progresión de la enfermedad en la mpMRI. A pesar de ello, la mayoría de los estudios seleccionados concluyeron que la mpMRI tenía un valor predictivo negativo para la progresión del cáncer de próstata y podría usarse para el seguimiento de pacientes con AV, con mejoría si se relacionaba con otras variables como los niveles de antígeno prostático específico. Conclusión: la mpMRI está ganando importancia en la evaluación de pacientes con cáncer de próstata, incluyendo la indicación y seguimiento de pacientes en VA. A pesar de algunas limitaciones, se puede utilizar junto con otros componentes para evaluar la progresión de la enfermedad.Introdução: Câncer de próstata é a segunda neoplasia maligna mais comum no sexo masculino, com aumento crescente de sua incidência devido melhoria nos métodos diagnósticos, como a ressonância magnética multiparamétrica (RMmp). Diante disso, novas modalidades terapêuticas, como a vigilância ativa (VA) que é indicada no câncer de próstata de baixo risco, vem ganhando espaço. Objetivo: Averiguar o uso da RMmp para acompanhamento de pacientes em vigilância ativa como tratamento do câncer de próstata. Métodos: Trata-se de uma revisão integrativa realizada através das bases SciELO, PubMed ®, BVS e LILACS. No total, foram encontrados 389 estudos, após aplicação dos critérios de inclusão e de exclusão restaram 17 artigos escolhidos. Resultados: Foram analisados 5.206 pacientes com câncer de próstata em 15 estudos, pois em 2 artigos selecionados não haviam amostra. Alguns estudos avaliaram que a RMmp não era suficiente para seguimento dos pacientes em VA com câncer de próstata, havendo indicação de complementação com biópsia mesmo quando não houvesse indicação de progressão de doença na RMmp. Apesar disso, a maioria dos estudos selecionados concluíram que a RMmp apresentava um valor preditivo negativo para progressão do câncer de próstata, podendo ser usada para seguimento nos pacientes em VA, com melhora caso seja relacionada a outras variáveis como níveis do antígeno específico da próstata. Conclusão: RMmp vem ganhando importância na avaliação dos pacientes com câncer de próstata, incluindo a indicação e o seguimento de pacientes em VA.
