Need to reconfigurate SUS financing in the face of austerity: a narrative review

Resumo: The financing of the Unified Health System (SUS) is an advance in public health in Brazil, appreciated by social security for the viability of the right and universal coverage to the services offered by this system. The high growth in costs due to the increase in technologies, changes in the demographic and epidemiological profile, judicialization and the austerity caused by Constitutional Amendment No. 29 and the coronavirus pandemic bring reflections on the ability to build new funding for the sustainability of SUS. The objective of this research was to identify the evidence in studies on the financing of this system for universal coverage of social security related to health in Brazil, through a narrative review of the literature, resulting in the analysis of 22 studies. Based on the results, we identified that the economic austerity measure and the costs of the pandemic impact the public deficit and undermine universal coverage and comprehensive health care for citizens. It is necessary to reconfigure the allocation of financial resources to guarantee the right to health services in the SUS. In addition, there is a need to change the culture and model of health management, implement quality issues and provide greater cooperation between federative entities in a solidary way with the strengthening of social participation.

El alto crecimiento de los costos por el aumento de tecnologías, los cambios en el perfil demográfico y epidemiológico, la judicialización y la austeridad provocada por la Enmienda Constitucional N° 29 y la pandemia del coronavirus traen reflexiones sobre la capacidad de construir nuevos financiamientos para la sostenibilidad del SUS. El objetivo de esta investigación fue identificar la evidencia en los estudios sobre el financiamiento de este sistema para la cobertura universal de la seguridad social relacionada con la salud en Brasil, a través de una revisión narrativa de la literatura, resultando en el análisis de 22 estudios. Con base en los resultados, identificamos que la medida de austeridad económica y los costos de la pandemia impactan en el déficit público y socavan la cobertura universal y la atención integral en salud de los ciudadanos. Es necesario reconfigurar la asignación de recursos financieros para garantizar el derecho a los servicios de salud en el SUS. Además, existe la necesidad de cambiar la cultura y el modelo de gestión en salud, implementar temas de calidad y brindar una mayor cooperación entre las entidades federativas de manera solidaria con el fortalecimiento de la participación social.O financiamento do Sistema Único de Saúde (SUS) é um avanço na saúde pública no Brasil, apreciado pela seguridade social para a viabilização do direito e da cobertura universal aos serviços ofertados por esse sistema. O alto crescimento dos custos pelo incremento das tecnologias, mudanças do perfil demográfico e epidemiológico, judicialização e a austeridade ocasionada pela Emenda Constitucional n° 29 e a pandemia do coronavírus trazem reflexões sobre a capacidade de construir um novo financiamento para a sustentabilidade do SUS. O objetivo desta pesquisa foi identificar as evidências em estudos sobre o financiamento desse sistema para a cobertura universal da seguridade social relativas à saúde no Brasil, por meio de uma revisão narrativa da literatura, resultando na análise de 22 estudos. Baseado nos resultados, identificamos que a medida de austeridade econômica e os custos com a pandemia impactam no déficit público e prejudicam a cobertura universal e a atenção integral da saúde ao cidadão. É necessária a reconfiguração da alocação dos recursos financeiros para a garantia do direito aos serviços de saúde no SUS.
