Obesity, diabetes and COVID-19: morbidity and mortality in a private hospital in western Paraná

Resumo: Objective: to quantitatively and qualitatively analyze patients hospitalized for COVID-19 at Hospital São Lucas, in the city of Cascavel - PR, correlating the prognosis of these patients with preexisting morbidities such as obesity and diabetes mellitus. Methods: This was a cross-sectional, retrospective, quantitative and qualitative exploratory research that collected data from January to June 2021. Data from medical records of 385 patients, admitted by COVID-19 and seen at Hospital São Lucas, were used. Results: in the 385 patients analyzed, 66% were male and 87.4% were older than 41 years. Diabetics were 16.9% of the sample. BMI indicated overweight or obesity in 74.9%. Most remained hospitalized from 4 to 14 days. About the radiological analysis, 91.3% had pulmonary involvement, which was from 26% to 75% of the parenchyma in 80.61%. Respiratory assistance was required by 94% - in the form of non-invasive mechanical ventilation for 56.9%, and invasive for 37.1. The mortality rate was 27.5% - age, male gender and demand for invasive mechanical ventilation being the most evident risk factors. Conclusion: Age, male gender and need for invasive mechanical ventilation were the worst prognostic factors for COVID-19. Diabetes mellitus was associated with greater lung impairment. BMI was not relevant in the analysis of outcomes, possibly due to the large number of overweight or obese patients in the sample.

Métodos: se trató de una investigación exploratoria transversal, seccional, retrospectiva, cuantitativa y cualitativa de coleta de datos de enero a junio de 2021. Foram utilizados dados de prontuários de 385 pacientes, internados por COVID-19 e atendidos no Hospital São Lucas. Resultados: en los 385 pacientes analizados, el 66% eran hombres y el 87,4% tenían más de 41 años. Los diabéticos constituían el 16,9% de la muestra. El IMC indicaba sobrepeso u obesidad en el 74,9%. La mayoría permaneció hospitalizada entre 4 y 14 días. En cuanto al análisis radiológico, el 91,3% presentó afectación pulmonar, que fue del 26% al 75% del parénquima en el 80,61%. La asistencia respiratoria fue requerida por el 94% - en forma de ventilación mecánica no invasiva para el 56,9%, e invasiva para el 37,1. La tasa de mortalidad fue del 27,5%, siendo la edad, el sexo masculino y la demanda de ventilación mecánica invasiva los factores de riesgo más evidentes. Conclusión: La edad, el sexo masculino y la necesidad de ventilación mecánica invasiva fueron los factores de peor pronóstico para el COVID-19. La diabetes mellitus se asoció a un mayor deterioro pulmonar. Assistência respiratória foi demandada por 94% - na forma de ventilação mecânica não invasiva para 56,9%, e invasiva para 37,1. A taxa de mortalidade foi de 27,5% - sendo idade, sexo masculino e demanda por ventilação mecânica invasiva seus fatores de risco mais evidentes. Conclusão: Idade, sexo masculino e necessidade de ventilação mecânica invasiva foram os fatores de pior prognóstico frente à COVID-19. O diabetes mellitus foi associado a maior comprometimento pulmonar.
