Execution of strategies and safe practices in the prevention of pressure injuries

Resumo: Pressure injury is tissue destruction and death as a result of unrelieved soft tissue compression located between bony prominence and hard surface, decreasing circulation and consequent decrease in blood, nutrients and oxygen. Nursing, as a science of care, must perform actions that do not allow the development of pressure injuries to the patient under their responsibility, with attention to those who have movement limitations. Considering this important role, this study aims to analyze the implementation of strategies and safe practices in the prevention of pressure injuries in two health units, one for urgency and emergency and the other in a hospital in a city in the center-west of São Paulo. This is a cross-sectional study with a quantitative approach, carried out with members of the Nursing team. Data collection took place in November 2019, with an instrument developed by the authors and the Pieper Knowledge Test, and were analyzed by descriptive and inferential analysis using ANOVA 1 factor. In general terms, the final result of the test showed a statistically significant difference according to professional training (p = 0.017) and the health unit in operation (p = 0.003). Even if over 60%, mid-level nursing professionals demonstrate the need to improve knowledge on this topicThe nurse and the nursing team play important roles within health institutions. This study demonstrates that knowledge, according to the nursing class, was over 60%, with greater domain by nurses. Continuing and permanent education actions aimed at the theme of pressure injury prevention must be instituted at local institutional levels, which will reflect on improving quality and safety in health care actions.

La enfermería, como ciencia de la atención, debe realizar acciones que no permitan el desarrollo de lesiones por presión al paciente bajo su responsabilidad, con atención a las personas con limitaciones de movimiento. Considerando este importante papel, este estudio tiene como objetivo analizar la implementación de estrategias y prácticas seguras en la prevención de lesiones por presión en dos unidades de salud, una de urgencia y emergencia y la otra en un hospital en una ciudad en el centro-oeste de São Paulo. Este es un estudio transversal con un enfoque cuantitativo, realizado con miembros del equipo de enfermería. La recopilación de datos tuvo lugar en noviembre de 2019, con un instrumento desarrollado por los autores y la Prueba de conocimiento Pieper, y se analizaron mediante análisis descriptivo e inferencial utilizando el factor ANOVA 1. En términos generales, el resultado final de la prueba mostró una diferencia estadísticamente significativa de acuerdo con la capacitación profesional (p = 0.017) y la unidad de salud en operación (p = 0.003). Incluso si más del 60%, los profesionales de enfermería de nivel medio demuestran la necesidad de mejorar el conocimiento sobre este tema. La enfermera y el equipo de enfermería juegan roles importantes dentro de las instituciones de salud. Este estudio demuestra que el conocimiento, según la clase de enfermería, fue superior al 60%, con mayor dominio por parte de las enfermeras. Las acciones de educación continua y permanente dirigidas al tema de la prevención de lesiones por presión deben instituirse a nivel institucional local, lo que reflejará la mejora de la calidad y la seguridad de las acciones de atención médica.A lesão por pressão é a destruição e morte tecidual como resultado de compressão não aliviada de tecido mole situado entre proeminência óssea e superfície dura, diminuindo a circulação e consequentemente a diminuição de sangue, nutrientes e oxigênio. À enfermagem, como ciência do cuidado, deve executar ações que não permitam o desenvolvimento das lesões por pressão ao paciente sob sua responsabilidade, com atenção aos que apresentam limitações de movimentos. Considerando esse importante papel, este estudo tem por objetivo analisar a execução de estratégias e práticas seguras na prevenção de lesão por pressão em duas unidades de saúde sendo uma de urgência e emergência e a outra em uma unidade hospitalar em um município do centro-oeste paulista. Trata-se de estudo transversal, de abordagem quantitativa, realizado com integrantes da equipe de enfermagem. A coleta dos dados ocorreu no mês novembro de 2019, com instrumento elaborado pelos autores e Teste de Conhecimento de Pieper, e foram analisadas por análise descritiva e inferencial por meio de ANOVA 1 fator. Em termos gerais, o resultado final do teste evidenciou diferença estatisticamente significante conforme a formação profissional (p=0,017) e a unidade de saúde de atuação (p=0,003). Mesmo que acima dos 60%, profissionais de grau médio em enfermagem demonstram necessidade de melhoria de conhecimento nesta temática. O enfermeiro e a equipe de enfermagem exercem papéis importantes dentro das instituições de saúde. Este estudo demonstra que o conhecimento, de acordo com a classe da enfermagem, foi superior a 60%, com maior domínio por enfermeiros.
