Public oral health policies for children: historical perspective, state of the art and future challenges

Resumo: | The present study analyzed the primary health care policies aimed at Pediatric Dentistry, identifying the trajectory, the formation of the political agenda and the challenges, from 1912 to 2020. The strategy of searching for legal documents was carried out in the digital collection of the Ministry of Health, in the Federal Official Gazette and in the Legislation Portal. The articles were searched in the VHL and SciELO, using the descriptors: public health policies, oral health, children and schoolchildren, associated with each other. 356 articles were found and, after analysis, 35 were selected to compose this study. In addition, 18 legal documents and secondary data from oral health surveys were used to compose this historical line. It was found that children's access to public dental care was prioritized by the incremental model, based on the curative and interventionist logic. However, with the adoption of an expanded conception of the health-disease process, there was a need to adapt scientific dentistry to a model that privileges prevention and health promotion, expressing itself through integral models, inversion of attention, attention early, until reaching the current model of universal and egalitarian character that affects the public sphere. Despite the increase in the offer of access to clinical and educational actions aimed at this public, the prevalence of the main oral health problems is still far from acceptable levels, and it is evident how the oral health policies aimed at children are subordinate to the will policy of successive governments, and such actions must be assimilated as State policy, resolving the variability resulting from changes in the political-democratic-governmental scenario. |

La estrategia de búsqueda de documentos legales se llevó a cabo en la colección digital del Ministerio de Salud. , en el Diario Oficial de la Federación y en el Portal de Legislación. Los artículos fueron buscados en la BVS y SciELO, utilizando los descriptores: políticas públicas de salud, salud bucal, niños y escolares, asociados entre sí. Se encontraron 356 artículos y, tras el análisis, se seleccionaron 35. Además, se utilizaron 18 documentos legales y datos secundarios de encuestas de salud bucal para componer esta línea histórica. Se encontró que el acceso de los niños a la atención odontológica pública fue priorizado por el modelo incremental, basado en la lógica curativa e intervencionista. Con la adopción de una concepción ampliada del proceso salud-enfermedad, se dio la implementación de un modelo que favorece la prevención y promoción de la salud, expresándose a través de modelos integrales, de inversión de atención, atención temprana, hasta llegar al modelo actual. de carácter universal e igualitario que tocan la esfera pública. Já os artigos, foram pesquisados na BVS e SciELO, utilizando os descritores: políticas públicas de saúde, saúde bucal, criança e escolares, associados entre si. Foram encontrados 356 artigos e, após análise, 35 foram selecionados. Além deles, 18 documentos legais e dados secundários dos levantamentos de saúde bucal foram utilizados para composição desta linha histórica. Verificou-se que o acesso das crianças ao atendimento público odontológico foi priorizado pelo modelo incremental, fundamentado na lógica curativa e intervencionista. Com a adoção de uma concepção ampliada do processo saúde-doença, houve a implantação de um modelo que privilegia a prevenção e promoção da saúde, expressando-se por meio de modelos integrais, de inversão da atenção, atenção precoce, até atingir o modelo atual de caráter universal e igualitário que tange a esfera pública.
