Molar-incisor hypomineralization: the importance of differential diagnosis to institute adequate treatment for patients affected by this condition

Resumo: | This study proposes to review the literature about the Molar-Incisor Hypomineralization (MIH) to identify the clinical characteristics and know how to differentiate them from other enamel anomalies to promote the correct diagnosis and treatment of these cases. An exploratory narrative bibliographic review was carried out through consultations in the SciELO, LILACS, PubMed, and BVS databases, based on articles published in Portuguese and English, without restriction to the year of publication. The 22 studies selected for this review, evaluated MIH as a qualitative defect in enamel development, with a not so defined systemic etiology, in which it is possible to observe chromatic or structural changes in the enamel of first molars and permanent incisors. Teeth affected by MIH have an enamel with well-defined opacity, with a white, yellow, or brown coloration, visually identified as an abnormality in their translucency. Soon after the eruption of the tooth element, episodes of post-eruptive fractures are commonly seen due to the characteristics of the enamel being porous, being fragile, and susceptible to fracture. Frequently, the clinical features of MIH can be confused with other developmental defects of the enamel, thus hindering the early diagnosis and an adequate intervention for its treatment. Therefore, knowledge about this anomaly allows the professional to establish a differential diagnosis, which will favor an effective treatment plan, thus avoiding tooth loss and the establishment of a condition of well-being and comfort for the patient affected with this condition. |

Se realizó una revisión bibliográfica narrativa de carácter exploratorio mediante consultas en las bases de datos SciELO, LILACS, PubMed y BVS, a partir de artículos publicados en portugués e inglés, sin restricción al año de publicación. Los 22 estudios seleccionados para esta revisión evaluaron el HMI como defecto cualitativo en el desarrollo del esmalte, de etiología sistémica no tan definida, en el que es posible observar cambios cromáticos o estructurales en el esmalte de primeros molares e incisivos permanentes. Los dientes afectados por HMI tienen un esmalte con opacidad bien definida, con una coloración blanca, amarilla o marrón, identificada visualmente como una anomalía en su translucidez. Poco después de la erupción del elemento dentario, se observan habitualmente episodios de fracturas post-eruptivas debido a que las características del esmalte son porosas, frágiles y susceptibles a fracturas. Con frecuencia, las características clínicas de la HMI pueden confundirse con otros defectos del desarrollo del esmalte, lo que dificulta el diagnóstico precoz y una adecuada intervención para su tratamiento. Por tanto, el conocimiento de esta anomalía permite al profesional establecer un diagnóstico diferencial, lo que favorecerá un plan de tratamiento eficaz, evitando así la pérdida de los elementos dentales afectados y el establecimiento de una condición de bienestar y comodidad para el paciente afectado por esta patología.O presente estudo propõe revisar a literatura sobre a Hipomineralização molar-incisivo (HMI), com o objetivo de identificar as características clínicas e saber diferenciá-las de outras anomalias do esmalte para promover o correto diagnóstico e tratamento destes casos. Os dentes afetados pela HMI apresentam um esmalte com opacidade bem demarcada, apresentando uma coloração branca, amarela ou castanha, identificados visualmente como uma anormalidade em sua translucidez. Logo após a irrupção do elemento dentário, é comumente visualizado episódios de fraturas pós-eruptivas pelas características do esmalte ser poroso, sendo frágil e suscetível à fratura. Frequentemente, as características clínicas da HMI podem ser confundidas com outros defeitos de desenvolvimento do esmalte, dificultando assim o diagnóstico precoce e uma intervenção adequada para o seu tratamento.
