Private Health Insurance and Plans Company Quality Performance Index: comparative analysis of the change in the evaluation methodology

Resumo: | Aim: To analyze comparatively the results of the Private Health Insurance and Plans Company Quality Performance Index (IDSS) in the evaluations of the base year 2016 and 2017, given the methodology changes in the Private Health Insurance and Plans Performance Evaluation Program (PQO). Methods: This is an observational, analytical, retrospective study that compared the performance results of private health companies in the old methodology, base year 2016, and in the new methodology, base year 2017, segregated by modality and size of the operator. The data were analyzed using the Wilcoxon test with R software. Results: It was found that the change in methodology significantly reduced the overall IDSS score with greater emphasis on three of the four dimensions, namely: Quality in Health Care, Guarantee of Access, and Management and Regulation. This reduction occurred among all sizes of private health companies, with the greatest amplitude among small companies, and significant reduction among all private health insurer type, except for private health insurer. Conclusion: The findings of this study indicate that the changes in the methodology impacted the performance score of the operators, especially the small ones. It suggests the need to understand the impacts on the supplementary health sector and the benefits of the methodology change for the health system. |

Métodos: Se trata de un estudio observacional, analítico, retrospectivo que comparó los resultados de desempeño de las empresas privadas de salud en la metodología antigua, año base 2016, y en la nueva metodología, año base 2017, segregados por modalidad y tamaño del operador. Los datos fueron analizados mediante la prueba de Wilcoxon con software R. Resultados: Se encontró que el cambio en la metodología redujo significativamente el puntaje general del IDSS con mayor énfasis en tres de las cuatro dimensiones, a saber: Calidad en la Atención de Salud, Garantía de Acceso y Gestión y Regulación. Esta reducción se produjo entre todos los tamaños de empresas privadas de salud, con la mayor amplitud entre las pequeñas empresas, y una reducción significativa entre todos los tipos de aseguradoras de salud privadas, excepto las aseguradoras de salud privadas. Conclusión: Los hallazgos de este estudio indican que los cambios en la metodología impactaron en el puntaje de desempeño de los operadores, especialmente los pequeños. Sugiere la necesidad de comprender los impactos en el sector de la salud suplementaria y los beneficios del cambio de metodología para el sistema de salud.Objetivo: analisar comparativamente os resultados do Índice de Desempenho da Saúde Suplementar (IDSS) nas avaliações do ano-base 2016 e 2017, frente às mudanças na metodologia do Programa de Qualificação das Operadoras (PQO). Métodologia: Estudo observacional analítico, de corte retrospectivo, que comparou os resultados de desempenho das operadoras de planos de saúde na metodologia antiga, ano-base 2016, e na nova metodologia, ano-base 2017, segregados por modalidade e porte da operadora. Os dados foram analisados por meio do teste de Wilcoxon a partir do software R. Resultados: Constatou-se que a mudança na metodologia reduziu significativamente a pontuação geral do IDSS com maior ênfase em três das quatros dimensões, a saber: Qualidade na Atenção à Saúde, Garantia de Acesso e Gestão e Regulação. Essa redução ocorreu entre todos os portes de operadoras, sendo a amplitude maior entre as de pequeno porte, com redução significativa entre todas as modalidades de operadoras, exceto as Seguradoras Especializadas em Saúde. Conclusão: Os achados deste estudo indicam que as mudanças na metodologia impactaram na pontuação de desempenho das operadoras, principalmente as de pequeno porte.
