Factors associated with the prevalence of exclusive breastfeeding to low weight neonate: integrative review

Resumo: Objective: Report the associated factors of exclusive breastfeeding for low weight newborns. Method: This is an integrative review, using journals in the databases: VHL and Scielo. Through exploratory reading, followed by selective reading, original and complete articles in the last five years were established as inclusion criteria. Excluding theses and dissertations, incomplete texts, editorial articles, abstracts, or publications outside the established period. Results and discussion: It was possible to verify that despite the various strategies to encourage exclusive breastfeeding for low weight newborns, such as the Human Milk Bank, Baby Friendly Hospital and the kangaroo method, many studies show that there is still a high percentage early weaning in Brazil, even in low birth weight infants. Some associated factors presented are maternal age, mother's return to the labor market, cesarean delivery, use of pacifiers and other liquids before the age of six months. Conclusion: Exclusive breastfeeding is essential for improving the health conditions of the newborn. Factors ranging from the type of delivery to the incentive by the health team can contribute to maintaining this breastfeeding. The importance of further studies on the effective methods of incentive and main reasons that lead to early weaning is highlighted.

Método: Esta es una revisión integradora, que utiliza revistas en las bases de datos: BVS y Scielo. Mediante la lectura exploratoria, seguida de la lectura selectiva, se establecieron artículos originales y completos en los últimos cinco años como criterios de inclusión. Excluyendo tesis y disertaciones, textos incompletos, artículos editoriales, resúmenes o publicaciones fuera del período establecido. Resultados y discusión: fue posible verificar que, a pesar de las diversas estrategias para alentar la lactancia materna exclusiva para los recién nacidos de bajo peso, como el Banco de Leche Humana, el Hospital Baby Friendly y el método canguro, muchos estudios muestran que todavía hay un alto porcentaje destete temprano en Brasil, incluso en bebés con bajo peso al nacer. Algunas razones presentadas son la edad materna, el regreso de la madre al mercado laboral, el parto por cesárea, el uso de chupetes y otros líquidos antes de los seis meses. Conclusión: La lactancia materna exclusiva es fundamental para mejorar las condiciones de salud del recién nacido. Factores que van desde el tipo de parto hasta el incentivo por parte del equipo de salud pueden contribuir a mantener esta lactancia. Se destaca la importancia de más estudios sobre los métodos efectivos de incentivo y las principales razones que conducen al destete temprano.Objetivo: Relatar os fatores associados à prevalência do aleitamento materno exclusivo ao neonato de baixo peso. Método: Trata-se de uma revisão integrativa, utilizando periódicos nas bases de dados: BVS e Scielo. Primeiro elaborou-se a pergunta norteadora “Quais fatores estão associados à prevalência do aleitamento materno exclusivo ao neonato de baixo peso?”; seguido pela busca na literatura, coleta de dados, análise crítica dos estudos incluídos, discussão dos resultados e por fim apresentação da revisão integrativa. Foram estabelecidos como critérios de inclusão artigos originais e completos nos últimos cinco anos.  Excluindo teses e dissertações, textos incompletos, artigos editoriais, resumos, ou publicações fora do período estabelecido. Resultados e discussão: Foi possível verificar que apesar das diversas estratégias de incentivo a amamentação exclusiva para recém-nascidos de baixo peso, como por exemplo, Banco de Leite Humano, Hospital Amigo da Criança e método canguru, muitos estudos mostram que ainda existe alto percentual de desmame precoce no Brasil, mesmo em neonatos de baixo peso. Alguns fatores associados a isso são a idade materna, volta ao mercado de trabalho pela mãe, parto cesariano, uso de chupetas e outros líquidos antes dos seis meses de idade. Conclusão: O Aleitamento materno exclusivo se mostra fundamental para melhoria das condições de saúde do recém-nascido. Fatores que vão desde o tipo de parto ao incentivo pela equipe de saúde podem contribuir para a manutenção desse aleitamento.
