O uso de Cannabis como fator de risco para o desenvolvimento/progressão de quadros de Esquizofrenia: Uma revisão de literatura

Resumo: Introduction: Cannabis is the third most used psychoactive substance in the world. Its use, in addition to other comorbidities, has an impact on mental health, with emphasis on schizophrenic conditions, since around 26.6% of these patients suffer from Cannabis Use Disorder (CUD). The use of this drug is frequent in patients with psychotic disorders, such as schizophrenia, and it also seems to stimulate isolated psychotic conditions. The objective of the work is to elucidate the association between the use of cannabis and its impact on the establishment/progression of schizophrenia Methodology: This is an integrative review on the impact of cannabis use in the context of schizophrenia. Data from the Virtual Health Library (VHL), Scientific Electronic Library Online (SciELO) and National Library of Medicine (PubMed) databases were used, by crossing the descriptors “Schizophrenia”, “Endocannabinoid system”, “Cannabis”, “THC” and “Cannabis psychosis”, to answer the question formulated through the PICO strategy. Results and discussion: The association between the two entities appears to be contained in a multifactorial genetic-behavioral relationship, in which early and exaggerated exposure to high concentrations of the drug, as well as family history and the polymorphism of some genes, appears to provide the individual with a condition of vulnerability to the earlier and more severe development of schizophrenia and/or psychotic events. Conclusion: The use of cannabis provides a more severe symptomatology, with early onset, with a high relapse rate, an early first hospital admission and recurrent psychotic conditions, this action being apparently provided by the interaction of THC with the endocannabinoid system.

Su consumo, además de otras comorbilidades, tiene un impacto en la salud mental, con énfasis en las condiciones esquizofrénicas, ya que alrededor del 26,6% de estos pacientes padecen Trastorno por Consumo de Cannabis (CUD). El uso de este fármaco es frecuente en pacientes con trastornos psicóticos, como la esquizofrenia, y también parece estimular cuadros psicóticos aislados. El objetivo del trabajo es dilucidar la asociación entre el consumo de cannabis y su impacto en el establecimiento/progresión de la esquizofrenia. Metodología: Se trata de una revisión integradora sobre el impacto del consumo de cannabis en el contexto de la esquizofrenia. Se utilizaron datos de las bases de datos Biblioteca Virtual en Salud (BVS), Biblioteca Electrónica Científica en Línea (SciELO) y Biblioteca Nacional de Medicina (PubMed), cruzando los descriptores “Esquizofrenia”, “Sistema Endocannabinoide”, “Cannabis”, “THC” y “Psicosis cannábica”, para responder a la pregunta formulada a través de la estrategia PICO. Resultados y discusión: La asociación entre ambas entidades parece estar contenida en una relación genético-conductual multifactorial, en la que la exposición temprana y exagerada a altas concentraciones de la droga, así como los antecedentes familiares y el polimorfismo de algunos genes, parecen proporcionar el individuo con una condición de vulnerabilidad al desarrollo más temprano y más severo de esquizofrenia y/o eventos psicóticos. Conclusión: El consumo de cannabis proporciona una sintomatología más severa, de inicio temprano, con una alta tasa de recaída, un primer ingreso hospitalario temprano y cuadros psicóticos recurrentes, siendo esta acción aparentemente proporcionada por la interacción del THC con el sistema endocannabinoide.Introdução: A Cannabis é a terceira substância psicoativa mais utilizada no mundo. Seu uso, além de outras comorbidades, tem impacto na saúde mental, com destaque para os quadros esquizofrênicos, uma vez que cerca de 26.6% desses pacientes sofrem do Transtorno pelo uso da Cannabis (TUC). O uso dessa droga é frequente em pacientes com desordens psicóticas, como a esquizofrenia e também parece estimular quadros psicóticos isolados. O objetivo do trabalho é elucidar a associação entre o uso da cannabis e seu impacto no estabelecimento/progressão da esquizofrenia Metodologia: Trata-se de uma revisão integrativa acerca do impacto do uso da Cannabis no contexto da esquizofrenia. Utilizou-se dados das bases Biblioteca Virtual de Saúde (BVS), Scientific Eletronic Library Online (SciELO) e National Library of Medicine (PubMed), a partir do cruzamento dos descritores “Schizophrenia”, “Endocannabinoid system”, “Cannabis”, “THC” e “Cannabis psychosis”, para responder à questão formulada através da estratégia PICO. Resultados e discussão: A associação entre as duas entidades parece estar contida numa relação multifatorial genético-comportalmental, em que a exposição precoce e exagerada a altas concentrações da droga, bem como o histórico familiar e o polimorfirmo de alguns genes, parece proporcionar ao indivíduo uma condição de vulnerabilidade ao desenvolvimento mais precoce e severo da esquizofrenia e/ou de eventos psicóticos.
