Cesarean childbirth in Brazil: prevalence, indications and risks entailed for the mother and child binomial

Resumo: This study aimed to understand the reasons why cesarean section is the most performed type of delivery in Brazil. This is an integrative literature review, whose data collection was carried out in the Virtual Health Library (VHL) and its indexed sites, having as keywords, after checking the Health Sciences Descriptors website (DeCS): cesarean section, neonatal morbidity and mortality and women's health; using the connective “and” to join them. The research had as inclusion criteria: studies published in the last 10 years, available in Portuguese and English, which were available in full and were free of charge; being excluded: materials with a publication date prior to 2012, which were available in languages other than Portuguese and English, incomplete, duplicated and paid for. After full reading, 10 studies were selected, which were analyzed, grouped and organized into two categories of discussion: factors related to the high rates of cesarean sections in Brazil and risks of cesarean section compared to vaginal delivery. The findings demonstrate the disproportionality of the percentage of cesarean sections recommended by the WHO and the current, in Brazil, that there are several factors related to this, and that the risks of the dissemination of this practice are real, but underestimated. This reality shows that it is essential to reduce this percentage and, for that, it is necessary to adopt a more humanized care model, in which the empowerment and autonomy of women are encouraged, and the dissemination of information about the risks and benefits of women delivery pathways.

Se trata de una revisión integrativa de la literatura, cuya recolección de datos se realizó en la Biblioteca Virtual en Salud (BVS) y sus sitios indexados, teniendo como palabras clave, previa consulta al sitio web Descriptores en Ciencias de la Salud (DeCS): cesárea, morbimortalidad neonatal y salud de la mujer. salud; usando el conectivo “y” para unirlos. La investigación tuvo como criterios de inclusión: estudios publicados en los últimos 10 años, disponibles en portugués e inglés, disponibles en su totalidad y sin costo; siendo excluidos: materiales con fecha de publicación anterior a 2012, que estuvieran disponibles en idiomas distintos al portugués e inglés, incompletos, duplicados y pagados. Después de la lectura completa, se seleccionaron 10 estudios, que fueron analizados, agrupados y organizados en dos categorías de discusión: factores relacionados con las altas tasas de cesáreas en Brasil y riesgos de la cesárea en comparación con el parto vaginal. Los hallazgos demuestran la desproporcionalidad del porcentaje de cesáreas recomendadas por la OMS y la actual, en Brasil, que existen varios factores relacionados con eso, y que los riesgos de la difusión de esta práctica son reales, pero subestimados. Esta realidad demuestra que es fundamental reducir este porcentaje y, para ello, es necesario adoptar un modelo de atención más humanizado, en el que se fomente el empoderamiento y la autonomía de las mujeres, y la difusión de información sobre los riesgos y beneficios de las mujeres vías de entrega.Este estudo objetivou entender que razões levam a cesariana a ser o tipo de parto mais realizado no Brasil. Trata-se de uma revisão integrativa da literatura, cuja coleta de dados foi realizada na Biblioteca Virtual em Saúde (BVS) e seus sites indexados, tendo como palavras-chave, após verificação no sítio dos Descritores em Ciências da Saúde (DeCS): cesárea, morbimortalidade neonatal e saúde da mulher; utilizando-se o conectivo “and” para uni-las. A pesquisa teve como critérios de inclusão: estudos publicados nos últimos 10 anos, disponíveis na língua portuguesa e inglesa, que se encontrassem disponibilizados na íntegra e que fossem gratuitos; sendo excluídos: materiais com data de publicação anterior a 2012, que estivessem disponíveis em línguas diferentes da portuguesa e inglesa, incompletos, duplicados e que fossem pagos. Após a leitura integral, foram selecionados 10 estudos, que foram analisados, agrupados e organizados em duas categorias de discussão: fatores relacionados às elevadas taxas de cesarianas no Brasil e riscos da cesariana em comparação ao parto normal. Os achados demonstram a desproporcionalidade da porcentagem de cesarianas preconizada pela OMS e a atual, no Brasil, que diversos são os fatores relacionados a isto, e que os riscos da disseminação dessa prática são reais, porém subestimados.
