Frailty burden in older adults with type 2 Diabetes Mellitus and related factors

Resumo: Aim: to determine the sociodemographic characteristics, clinica, functional and depressive symptoms related to the frailty burden in community-dwelling older adults with T2DM. Methods: observational, analytical cross-sectional study with diabetic older adults, aged ≥ 60 years, who are ambulatory and non-amputated. Sociodemographic characteristics, clinical-functional, and depressive symptoms and frailty phenotype were evaluated. A stepwise multivariate logistic regression analysis (p<0,05 and 95% Confidence Interval) was conducted. Results: The sample consisted of 125 participants (63,2% women) with a mean age of 68,7 ± 6,5 years. Nearly half the participants met the criteria for the lowest frailty load group, and the low physical activity level was most prevalent (72,0%). The final regression model showed that frailty burden was significantly associated with age (p=0,016; [1,316-8,794]), education (p=0,002; [1,680-10,623]), lower limb pain (p<0,001; [1,935-11,766]) and TUGT (p=0,031; [1,145-15,659]). This model presented 80.9% accuracy. Conclusions: Older adult patients with T2DM who present high frailty burden are in the age group of 70 years or more, use 5 or more medications, have had T2DM diagnosis for 6 years or more, have altered glycated hemoglobin and fasting glycemia, have longer walking and show higher depressive symptoms. Older people with lower frailty load have higher education and income.

Metodología: estudio observacional, analítico, transversal, con ancianos diabéticos, ≥ 60 años, ambulatorios y no amputados. Se evaluaron características sociodemográficas, síntomas clínico-funcionales, depresión y fenotipo de fragilidad. Se realizó un análisis de regresión logística multivariante (p <0.05 e intervalo de confianza del 95%). Resultados: La muestra consistió en 125 participantes (63,2% mujeres), con una edad promedio de 68,7 ± 6,5 años. Casi la mitad de los participantes cumplieron los criterios para el grupo con la carga de fragilidad más baja y el nivel más bajo de actividad física fue el más frecuente (72.0%). El modelo de regresión final mostró que la carga de fragilidad se asoció significativamente con la edad (p = 0.016; [1.316-8.794]), educación (p = 0.002; [1.680-10.623]), dolor en las extremidades inferiores (p <0.001; [1,935-11,766]) y movilidad funcional (p = 0,031; [1,145-15,659]). Este modelo mostró una precisión del 80.9%. Conclusiones: Los pacientes adultos mayores con DM2 que tienen una carga de fragilidad alta están en el grupo de edad de 70 años o más, usan 5 o más medicamentos, han sido diagnosticados con DM2 durante 6 años o más, tienen hemoglobina glucosilada alterada y glucemia en ayunas, caminan más y muestran síntomas depresivos más altos. Quase metade dos participantes atendeu aos critérios para o grupo com menor carga de fragilidade e o baixo nível de atividade física foi o mais prevalente (72,0%). O modelo de regressão final mostrou que a carga de fragilidade esteve significativamente associada à idade (p=0,016; [1.316-8.794]), escolaridade (p=0,002; [1.680-10.623]), dor nos membros inferiores (p<0,001; [1.935-11.766]) e mobilidade funcional (p=0,031; [1.145-15.659]). Este modelo apresentou 80,9% de precisão. Conclusões: Pacientes adultos mais velhos com DM2 que apresentam alta carga de fragilidade estão na faixa etária de 70 anos ou mais, usam 5 ou mais medicamentos, têm diagnóstico de DM2 há 6 anos ou mais, apresentam hemoglobina glicada e glycemia de jejum alteradas, caminham mais e mostram sintomas depressivos mais altos.
