Evaluation of MTA insertion techniques in apically prepared teeth simulating incomplete root formation: an in vitro study

Resumo: | Apexification with MTA in incomplete rhizogenesis allows the formation of the apical barrier quickly and safely, however, there is difficulty in its insertion in the root canals. In this study, the efficacy of apical sealing in human teeth was comparatively evaluated in vitro, when the devices: MTA carrier, Lentulo spiral and gutta-percha cone were used. Sixty extracted anterior teeth were used, with lengths between 20 and 25 mm. The preparation of root canals was carried out by using Gates-Glidden drills, progressively, from number 1 to number 4, and for the simulation of apical divergence, the 5 was used, in a retrograde fashion. The sample was randomized into 3 groups (n=20) to make the apical plugs. Group 01, Lentulo, group 2, the MTA carrier and group 3, gutta-percha #80. The quality of the apical barriers was evaluated by a resident in Endodontics, through digital periapical radiographs. A score from 0 to 3 was considered, verifying the correspondence of the apical plug to the radiographic apex, the filling of the apical 4 mm of the root, and the absence of space between the material and the canal walls. The results showed that there was no significant difference between the groups (p> 0.05) and the scores in descending order were: 2 (48.3%), 1 (23.3%), 3 (18.4%) and 0 (10%). It was concluded that there was no difference in the effectiveness of apical sealing in human teeth, in vitro, simulating incomplete rhizogenesis, when compared to the Lentulo spiral, the MTA carrier, and gutta percha cone. |

En este estudio in vitro se evaluó comparativamente la eficacia del sellado apical en dientes humanos, cuando se utilizaron los dispositivos: portador de MTA, espiral Lentulo y cono de gutapercha. Se utilizaron 60 dientes anteriores extraídos, con longitudes entre 20 y 25 mm. La preparación de los conductos radiculares se realizó con fresas de Gates-Glidden, progresivamente, del número 1 al 4, y para la simulación de la divergencia apical se utilizó la fresa número 5, en forma retrógrada. La muestra se aleatorizó en 3 grupos (n=20) para realizar los tapones apicales. Grupo 1, Lentulo, grupo 2, porta-MTA y grupo 3, gutapercha #80. La calidad de las barreras apicales fue evaluada, a través de radiografías periapicales digitales. Se consideró una puntuación de 0 a 3, verificando la correspondencia del tapón apical con el ápice radiográfico, el relleno de los 4 mm apicales de la raíz y la ausencia de espacio entre el material y las paredes del conducto. Los resultados mostraron que no hubo diferencia significativa entre los grupos (p> 0,05) y los puntajes en orden descendente fueron: grupo 2 (48,3%), 1 (23,3%), 3 (18,4%) y 0 (10%). Se concluyó que no hubo diferencia en la efectividad del sellado apical en dientes humanos in vitro, simulando rizogénesis incompleta, cuando se comparó con la espiral de Lentulo, el portador de MTA y el cono de gutapercha.A apicificação com MTA na rizogênese incompleta permite a formação da barreira apical de forma rápida e segura, contudo, existe a dificuldade da sua inserção nos condutos radiculares. Neste estudo avaliou-se comparativamente, in vitro, a eficácia do selamento apical em dentes humanos, quando utilizados os dispositivos: porta-MTA, broca Lentulo e cone de guta percha. Foram utilizados 60 dentes anteriores extraídos, com comprimentos entre 20 e 25 mm. O preparo dos canais radiculares deu-se pelo uso das brocas Gates-Glidden, progressivamente, do número 1 ao número 4, e para a simulação da divergência apical, usou-se a 5, de forma retrógrada. A amostra foi randomizada em 3 grupos (n=20) para a confecção dos plugs apicais. Grupo 01, broca Lentulo, grupo 2, o porta-MTA e o grupo 3, cone de guta-percha #80. A qualidade das barreiras apicais passou pela avaliação de um residente em Endodontia, por meio de radiografias periapicais digitais. Considerou-se um escore de 0 a 3, verificando a correspondência do plug apical ao ápice radiográfico, o preenchimento dos 4 mm apicais da raiz e a ausência de espaço entre o material e as paredes do canal. Os resultados mostraram que não houve diferença significativa entre os grupos (p> 0,05) e os escores em ordem decrescente foram: 2 (48,3%), 1 (23,3%), 3 (18,4%) e 0 (10%).
