Botulinum toxin versus others alternative methods as an option for the management of patients with bruxism: Systematic review

Resumo: Objective: To determine if botulinum toxin is effective as a treatment for adult patients diagnosed with bruxism according to RCT-type scientific articles that have been published from 2016 to 2021. Methodology: An electronic search was carried out between November 2021 and January 2022 through three digital databases (PubMed, Science Direct, Cochrane Library), using the keywords "Bruxism", "¨Teeth grinding", "Botulinum toxin" , “Botox”, “Botulinum toxin A”, “Botulinum toxin type A” combined through the Boolean equation “AND”. For this review, the PRISMA scientific tool was considered, articles published in the last 5 years, in English and Spanish, in adult patients with bruxism, RCT type and prospective comparing Botox with placebo or traditional methods were included. Results: Of 90 screened articles, 6 RCTs were included in the review. Of these, 3 studies compared the effectiveness of BTX with occlusal splints and the remaining 3 with placebo. No significant differences were found between both methods and a decrease in symptoms and episodes of bruxism was observed. Control sessions were carried out up to 12 months without reporting adverse effects. Conclusions: BTX and traditional methods are appropriate for the treatment of bruxism because they reduce symptoms and prevent the appearance or progression of clinical signs. Nevertheless; considering that the pathology is multifactorial, the ideal treatment should be multidisciplinary, including: psychological and neurological treatment, physical therapy, pharmacological treatment, BTX applications combined with occlusal splints and restorative treatments.

Metodología: Se realizó una búsqueda electrónica entre noviembre del 2021 a enero del 2022 a través de tres bases digitales (PubMed, Science Direct, Cochrane Library), utilizando las palabras claves “Bruxismo”, “¨Rechinar de dientes”, “Toxina botulínica”, “Botox”, “Toxina botulínica A”, “Toxina botulínica tipo A” combinadas a través de la ecuación booleana “AND”. Para esta revisión se consideró la herramienta científica PRISMA, se incluyeron artículos publicados en los últimos 5 años, en idioma inglés y español, en pacientes adultos con bruxismo, tipo ECA y prospectivos comparando el botox con placebo o métodos tradicionales. Resultados: De 90 artículos cribados, 6 ECA fueron incluidos en la revisión. De estos, 3 estudios compararon la efectividad del BTX frente a las férulas oclusales y los 3 restantes con placebo. No se encontraron diferencias significativas entre ambos métodos y se observó una disminución de la sintomatología y los episodios de bruxismo. Se realizaron sesiones de control hasta los 12 meses sin reportarse efectos adversos. Conclusiones: El BTX y los métodos tradicionales son apropiados para el tratamiento del bruxismo ya que disminuyen la sintomatología y previenen la aparición o el avance de los signos clínicos. Sin embargo; considerando que la patología es multifactorial, el tratamiento idóneo debería ser multidisciplinario abarcando: tratamiento psicológico y neurológico, terapia física, tratamiento farmacológico, aplicaciones de BTX combinado con férulas oclusales y tratamientos restaurativos.Objetivo: Determinar se a toxina botulínica é eficaz como tratamento para pacientes adultos diagnosticados com bruxismo de acordo com artigos científicos do tipo ECR publicados de 2016 a 2021. Metodologia: Foi realizada uma busca eletrônica entre novembro de 2021 e janeiro de 2022 através de três bases de dados digitais (PubMed, Science Direct, Cochrane Library), utilizando as palavras-chave "Bruxism", "¨Teeth grind", "Botulinum toxin", “Botox”, “Toxina botulínica A”, “Toxina botulínica tipo A” combinadas através da equação booleana “AND”. Para esta revisão foi considerada a ferramenta científica PRISMA, foram incluídos artigos publicados nos últimos 5 anos, em inglês e espanhol, em pacientes adultos com bruxismo, tipo ECR e prospectiva comparando Botox com placebo ou métodos tradicionais. Resultados: Dos 90 artigos selecionados, 6 ECRs foram incluídos na revisão. Destes, 3 estudos compararam a eficácia da BTX com placas oclusais e os 3 restantes com placebo. Não foram encontradas diferenças significativas entre os dois métodos e foi observada diminuição dos sintomas e episódios de bruxismo. As sessões de controle foram realizadas até 12 meses sem relatar efeitos adversos. Conclusões: A BTX e os métodos tradicionais são adequados para o tratamento do bruxismo, pois reduzem os sintomas e previnem o aparecimento ou progressão dos sinais clínicos.
