Use of antibiotics in renal failure: need for dose adjustments and individualized doses

Resumo: | Pharmacokinetic and pharmacodynamic changes in medicinal products due to impaired renal function are frequently observed. According to the World Health Organization -WHO (2015), it is possible to lose up to 90% of renal function before manifesting any symptoms. The aim of this study was to create a practical table for consultation of antibiotics that need adjustment in patients with renal failure. A documentary, descriptive and analytical study with a qualitative approach was performed based on the pharmacokinetic analysis of standardized antibiotics at the University Hospital of Piaui, seeking to identify the need for dose adjustments and frequency of use according to the renal function of the patient. For the execution of the study, references described in package inserts for health professionals were consulted, and the information compared to references described in pharmacotherapeutic guides of the main excellence hospitals in Brazil and the Standford guide, as well as literature reviews. Antimicrobials were organized by pharmacological class, describing dose adjustment depending on creatine clearance and supplemental dose after hemodialysis. Despite the guidelines regarding dosage adjustment in patients with renal failure, there are still doubts, as there are other factors that influence this correction such as the dialysis method used in hemodialysis that includes: intermittent or conventional, ultrafiltration, low efficiency dialysis. The present study generated a quick consultation table to clarify doubts in the prescription or dispensation of standardized antimicrobials in the hospital, thus promoting safer and more efficient antimicrobial therapy. |

Según la Organización Mundial de la Salud - OMS (2015), es posible perder hasta el 90% de la función renal antes de manifestar algún síntoma. El objetivo de este trabajo fue crear una mesa práctica de consulta de antibióticos que necesiten ajustes en pacientes con insuficiencia renal. Se realizó un estudio documental, descriptivo y analítico con enfoque cualitativo basado en el análisis de la farmacocinética de antibióticos estándar en el Hospital Universitario do Piaui, buscando identificar la necesidad de ajustes en las dosis y frecuencia de uso de acuerdo con la función renal del paciente. Para la ejecución del estudio se consultaron las referencias descritas en los prospectos para los profesionales de la salud, comparándose la información con las referencias descritas en las guías farmacoterapéuticas de los principales hospitales de excelencia de Brasil según el Ministerio de Salud, y la guía Standford, además de las revisiones de la literatura. Los antimicrobianos se organizaron por clase farmacológica, describiendo el ajuste de la dosis, según el aclaramiento de creatina, y la dosis suplementaria después de la hemodiálisis. A pesar de las pautas de ajuste de dosis en pacientes con insuficiencia renal, quedan dudas, ya que existen otros factores que influyen en esta corrección, como el método de diálisis utilizado en hemodiálisis, que comprende: intermitente o convencional, ultrafiltración, diálisis de baja eficacia. El presente trabajo generó una mesa de consulta rápida con el fin de aclarar dudas en la prescripción o dispensación de antimicrobianos estandarizados en el hospital, promoviendo así una terapia antimicrobiana más segura y eficiente.A alteração farmacocinética e farmacodinâmica de medicamentos, em virtude de comprometimento da função renal, é frequentemente observada. Segundo a Organização Mundial da Saúde - OMS (2015), é possível perder até 90% da função renal antes de manifestar qualquer sintoma. O objetivo deste trabalho foi confeccionar uma tabela prática para consulta de antibióticos que necessitam de ajustes em pacientes com insuficiência renal. Realizou-se um estudo documental, descritivo e analítico com abordagem qualitativa a partir da análise da farmacocinética dos antibióticos padronizados do Hospital Universitário do Piauí buscando identificar a necessidade de ajustes de doses e frequências de uso conforme a função renal do paciente. Para a execução do estudo foram consultadas referências descritas em bulas para profissionais da saúde, sendo as informações comparadas às referências descritas em guias farmacoterapêuticos dos principais hospitais de excelência do Brasil segundo o Ministério da Saúde, e o guia Standford, além de revisões de literatura. Os antimicrobianos foram organizados por classe farmacológica, descrevendo o ajuste da dose, a depender do clearence de creatina, e dose suplementar após hemodiálise. Apesar das orientações a respeito do ajuste posológico em pacientes com insuficiência renal, permanecem ainda dúvidas, pois existem outros fatores que influenciam nessa correção como o método dialítico empregado na hemodiálise que compreende: intermitente ou convencional, ultrafiltração, diálise de baixa eficiência.
