Surgical treatment of severely displaced atrophic jaw fracture: a case report

Resumo: Atrophic mandible fractures usually occur in elderly individuals, in which osteogenesis is decreased, cortical bone is dense and blood supply is decreased. There is a consensus in the literature that the treatment of atrophic mandible fractures remains a major challenge for maxillofacial traumatology, considering the low availability of bone and the morphological and biomechanical characteristics of a toothless mandible. This paper aims to report a case of surgical treatment of bilateral atrophic mandible fracture. The male patient, 61 years old, attended the Oral and Maxillofacial Surgery service, after being the victim of a motorcycle accident, without the use of individual protection materials, evolving with bilateral mandible fracture. On physical examination, the patient had diastasis in the bilateral mandibular body area and in the intraoral examination, ecchymosis was observed in the floor of the mouth. The treatment consisted of an extraoral surgical approach, under general anesthesia, transcervical access, reduction and osteosynthesis of fractured jaw with a 2.4mm locking system plate in each fracture. The therapy allowed the patient to resume his chewing functions with a gain in quality of life. It is concluded that atrophic mandible fractures are uncommon, but with the increase in the elderly population in the coming years, it is expected that its incidence will increase, and it is essential for the maxillofacial surgeon to recognize the available treatment possibilities.

Existe un consenso en la literatura de que el tratamiento de las fracturas de la mandíbula atrófica sigue siendo un desafío importante para la traumatología maxilofacial, dada la baja disponibilidad de hueso y las características morfológicas y biomecánicas de una mandíbula sin dientes. Este artículo tiene como objetivo informar un caso de tratamiento quirúrgico de fractura mandibular atrófica bilateral. El paciente masculino, de 61 años, asistió al servicio de Cirugía Oral y Maxilofacial, luego de ser víctima de un accidente de motocicleta, sin el uso de materiales de protección individual, evolucionando con fractura mandibular bilateral. En el examen físico, el paciente tenía diástasis en el área bilateral del cuerpo mandibular y en el examen intraoral, se observó equimosis en la región del piso vestibular. El tratamiento consistió en un abordaje quirúrgico extraoral, bajo anestesia general, acceso transcervical, reducción y osteosíntesis de muñones fracturados con una placa de sistema de bloqueo de 2.4 mm en cada fractura. La terapia permitió al paciente reanudar sus funciones de masticación con un aumento en la calidad de vida. Se concluye que las fracturas de la mandíbula atrófica son poco frecuentes, pero con el aumento de la población de edad avanzada en los próximos años, se espera que su incidencia aumente y es esencial que el cirujano maxilofacial conozca y conozca las posibilidades de tratamiento disponibles.As fraturas de mandíbulas atróficas geralmente ocorrem em indivíduos idosos, nos quais a osteogênese está diminuída, osso cortical é denso e suprimento sanguíneo diminuído. É consenso na literatura que o tratamento das fraturas de mandíbula atrófica permanece como um grande desafio para a traumatologia maxilofacial, tendo em vista a baixa disponibilidade de osso e as características morfológicas e biomecânicas de uma mandíbula desdentada. Este trabalho tem como objetivo relatar um caso de tratamento cirúrgico de fratura bilateral de mandíbula atrófica. O paciente do gênero masculino, 61 anos de idade, compareceu ao serviço de Cirurgia e Traumatologia Buco-Maxilo-Facial, após ter sido vítima de acidente motociclístico, sem o uso dos materiais de proteção individual, evoluindo com fratura bilateral de mandíbula. Ao exame físico, o paciente apresentava diástase em área de corpo mandibular bilateral e no exame intraoral foi observado equimose na região de assoalho bucal. O tratamento consistiu em uma abordagem cirúrgica extraoral, sob anestesia geral, acesso transcervical, redução e osteossíntese dos cotos fraturados com uma placa do sistema 2.4mm locking em cada uma das fraturas. A terapia proporcionou ao paciente retomada de suas funções de mastigação com ganho em qualidade de vida.
