Analysis of the deglutition parameters in patients with dysphagia after speech therapy and nutritional support

Resumo: Dysphagia is a change in swallowing that can be caused by underlying diseases that lead to structural and functional changes or by inappropriate habits of the individual when eating. This condition can lead to a great compromise in the patient's quality of life, which may result in future feeding through alternative routes, as well as being associated with bronchial aspiration and malnutrition. Therefore, it is evident the need to study the evolution of these patients through fiberoptic endoscopic examination of swallowing (FESS) after phonotherapy and nutritional support. Thus, a retrospective analysis of patients with FESS was performed before and after support by speech therapy and nutritional therapy to characterize posterior escape, Yale Classification, Penetration and Aspiration. The research resulted from an evaluation of 16 patients, in which 37.5% were observed after treatment with normal swallowing or with reduced amount of waste. The maintenance of the parameters evaluated between the two FESS were the most prevalent findings in most of the analyzed variables. Thus, therapy could indicate a potential stabilizer of patients' dysphagic evolution.

Esta condición puede conllevar un gran compromiso en la calidad de vida del paciente, lo que puede derivar en una futura alimentación por vías alternativas, además de asociarse a broncoaspiración y desnutrición. Por tanto, es evidente la necesidad de estudiar la evolución de estos pacientes mediante examen endoscópico de fibra óptica de la deglución (FESS) tras la fonoterapia y el soporte nutricional. Por lo tanto, se realizó un análisis retrospectivo de pacientes con FESS antes y después del apoyo de logopedia y nutrición para caracterizar el escape posterior, la Clasificación de Yale, la Penetración y la Aspiración. La investigación resultó de una evaluación de 16 pacientes, en los que el 37,5% se observó después del tratamiento con deglución normal o con cantidad reducida de desechos. El mantenimiento de los parámetros evaluados entre las dos FESS fueron los hallazgos más prevalentes en la mayoría de las variables analizadas. Por tanto, la terapia podría indicar un posible estabilizador de la evolución disfágica de los pacientes.A disfagia é uma alteração da deglutição que pode ser causada por doenças subjacentes as quais levam a alterações estruturais e funcionais ou por hábitos inadequados do indivíduo ao alimentar-se. Essa condição pode levar a um grande comprometimento na qualidade de vida do paciente, podendo resultar em futuras alimentações por vias alternativas, bem como pode ser associada a quadros de broncoaspiração e desnutrição. Diante disso, evidencia-se a necessidade de estudar a evolução desses pacientes através da Vídeoendoscopia da Deglutição (VED) após suporte fonoterápico e nutricional. Dessa forma, realizou-se análise retrospectiva de pacientes com VED antes e após suporte por terapia fonoaudiológica e nutricional para caracterizar escape posterior, Classificação de Yale, Penetração e Aspiração. A pesquisa resultou de uma avaliação de 16 pacientes, em que foi observado após tratamento 37,5% com deglutição normal ou com redução da quantidade de resíduos. A manutenção dos parâmetros avaliados entre as duas VED foram os achados mais prevalentes na maioria das variáveis analisadas.
