Resumo: Most cases of head and neck cancer (HNC) originate from squamous cells and include tumors that arise in the epithelium of the nasal and oral cavity, paranasal sinuses, pharynx and larynx. More than half a million cases are diagnosed each year. The treatment for this neoplasm is considered a challenge, due to its intratumoral heterogeneity and the various anatomical subsites. In addition to genetic factors, it is also necessary to consider environmental components and gene-environment interactions, which can induce susceptibility to the disease. The main focus of therapies is to decrease side effects as well as increase the overall survival of patients. Due to the great complexity in tumor heterogeneity, a better understanding of the epigenetic mechanisms is of paramount importance for the development of therapies for this neoplasm. Given this context, this work aims to carry out a literature review regarding the genetics of head and neck cancer, as well as advances in molecular research. A total of 74 articles were selected, which provide a comprehensive view on the development of research aimed at clinical applications in diagnosis, prevention and treatment, whose research source was PubMed. According to the literature, the elucidation of the epigenetic and genetic mechanisms associated with carcinogenesis have a relevant role in the progression and response to treatment, the understanding of their participation may provide insights into their relationship in tumorigenesis, which are essential for precision medicine, contributing to personalized and patient-directed therapies.

Cada año se diagnostican más de medio millón de casos. El tratamiento de esta neoplasia se considera un desafío, dada su heterogeneidad intratumoral y las diversas subsecciones anatómicas. Además de los factores genéticos, también es necesario considerar los componentes ambientales y las interacciones gen-ambiente, que pueden inducir la susceptibilidad a la enfermedad. El objetivo principal de las terapias es disminuir los efectos secundarios y aumentar la supervivencia general de los pacientes. Debido a la gran complejidad en la heterogeneidad tumoral, una mejor comprensión de los mecanismos epigenéticos es de suma importancia para el desarrollo de terapias para esta neoplasia. Ante este contexto, este trabajo tiene como objetivo realizar una revisión bibliográfica respecto a la genética del cáncer de cabeza y cuello, así como los avances en la investigación molecular. Se seleccionaron un total de 74 artículos, que brindan una visión integral sobre el desarrollo de investigaciones orientadas a aplicaciones clínicas en diagnóstico, prevención y tratamiento, cuya fuente de investigación fue PubMed. De acuerdo con la literatura, la elucidación de los mecanismos epigenéticos y genéticos asociados a la carcinogénesis tienen un papel relevante en la progresión y respuesta al tratamiento, la comprensión de su participación puede proporcionar conocimientos sobre su relación en la tumorigénesis, que son esenciales para la medicina de precisión, contribuyendo a terapias personalizadas y dirigidas al paciente.Grande parte dos casos de câncer de cabeça e pescoço (CCP), são originários das células escamosas e inclui tumores que surgem no epitélio da cavidade nasal e oral, seios paranasais, faringe e laringe. Mais de meio milhão de casos são diagnosticados por ano. O tratamento para essa neoplasia é considerado um desafio, devido sua heterogeneidade intratumoral e os vários subsítios anatômicos. Além dos fatores genéticos, é necessário levar em consideração também os componentes ambientais e interações gene-ambiente, os quais podem induzir a suscetibilidade à doença. O principal foco das terapias, é diminuir os efeitos colaterais, bem como aumentar a sobrevida global dos pacientes. Devido à grande complexidade na heterogeneidade tumoral, o maior entendimento dos mecanismos epigenéticos é de suma importância para o desenvolvimento de terapêuticas para essa neoplasia. Diante deste contexto, este trabalho tem como objetivo realizar uma revisão de literatura quanto à genética do CCP, bem como os avanços na pesquisa molecular. Foram selecionados 74 artigos, que fornecem uma visão abrangente sobre o desenvolvimento de pesquisas voltadas para aplicações clínicas em diagnóstico, prevenção e tratamento, cuja fonte de pesquisa foi o PubMed.
