Resumo: Objective:  identify the categories that should be part of the safe discharge plan for elderly patients with chronic diseases, which will serve as a guiding document for the federal hospital units in Rio de Janeiro, aiming to contribute to the implementation of best practices in the processes of dehospitalization and transition from hospital care to primary health care. Methodology: this is an exploratory descriptive study with a qualitative approach, that uses the focus group technique and Thematic Content Analysis with IRAMUTEQ software. Results: the data analysis provided the identification of categories considered important when drawing up a safe discharge plan, which were: admission and gateway, planning for safe discharge, patient-centered care and health care education and training. Remarks: The content learned from the research indicates the need for hospital health services to develop actions, protocols and plans for safe discharge of patients. It is necessary to promote a change in organizational culture in order to advance in patient-centered care based on the integrality of care.

Desde la atención al alta hospitalaria hasta la Atención Primaria de Salud Metodología: se trata de un estudio exploratorio descriptivo con abordaje cualitativo, utilizando la técnica de grupos focales y Análisis de Contenido Temático. Resultados: el análisis de los datos permitió identificar las categorías consideradas importantes para componer el plan de altos responsables, fueron: recepción y entrada, planificación de altos responsables, atención centrada en la persona y educación en salud. Consideraciones: El contenido aprehendido en la investigación demuestra la necesidad de que los servicios de salud hospitalarios desarrollen acciones, desarrollen protocolos y planes para la alta dirección y que es necesario invertir en un cambio de cultura institucional para avanzar en la atención centrada en el paciente basada en la atención integral.Objetivo: identificar as categorias que deverão compor o plano para alta responsável de pacientes idosos com doenças crônicas e que servirá de documento norteador para as unidades hospitalares federais no Rio de Janeiro, visando contribuir para a realização de melhores práticas nos processos de desospitalização e transição do cuidado do hospital para a Atenção Primária à Saúde. Metodologia: estudo descritivo exploratório com abordagem qualitativa, com uso da técnica de grupo focal e Análise Temática de Conteúdo, com auxílio do software IRAMUTEQ. Resultados: a análise dos dados permitiu a identificação das categorias consideradas importantes para compor o plano para alta responsável, a saber: acolhimento e porta de entrada, planejamento para alta responsável, atenção centrada na pessoa e educação em saúde. Considerações: O conteúdo apreendido na pesquisa demonstra a necessidade dos serviços de saúde hospitalares desenvolverem ações, elaborarem protocolos e planos para alta responsável.
