Prevalence of reactivity to total anti-hbc (IgM, IgG) in blood donation candidates in a region of northeastern Brazil

Resumo: Over the past few years, several advances have taken place in transfusion medicine, as well as the development of laboratory techniques used in the serological screening of blood donors, aiming to minimize the transmission of infectious diseases. The importance of detecting total anti-HBc aims to aid in the differential diagnosis of hepatitis, following hepatitis B virus (HBV) infection together with other viral markers, being used in blood banks as a screening test for blood donors, showing efficacy in detecting prior contact with HBV during the “negative window” of HBV. The aim of this study was to evaluate the serological profile for hepatitis B (total anti-HBc) in blood donation candidates in a region of northeastern Brazil. A cross-sectional study was carried out, of qualitative approach, with the computerization system of the entire blood donation cycle, with retrospective data and categorized according to sex, age, location, level of education and risk behavior, having as target population candidates for blood donation in the period from January 2016 to December 2020. Of the 136,652 candidates for donation, reactivity to total anti-HBc was observed in 2.69% (N=3,681). There was greater reactivity to total anti-HBc among male candidates 2.70% (n=2,912) and aged between 29 and 39 years, with 54.9% (n=2,024) living in the interior of the state and one total of 0.91% (n=1,247) with incomplete elementary school. Clinical, laboratory and serological pre-screening for the hepatitis B virus in blood donation candidates is a safety protocol for the transfusion procedure. Aiming to increase the reduction of the risks of contamination and loss of supplies used for the donation, it is necessary to mobilize vaccination campaigns for the hepatitis B virus, and for all those who apply to donate blood at Blood Centers.

La importancia de detectar el anti-HBc total tiene como objetivo ayudar en el diagnóstico diferencial de la hepatitis, después de la infección por el virus de la hepatitis B (VHB) junto con otros marcadores virales, siendo utilizado en bancos de sangre como prueba de cribado para donantes de sangre, demostrando eficacia en la detección previa. contacto con el VHB durante la "ventana negativa" del VHB. Hubo mayor reactividad al anti-HBc total entre los candidatos masculinos 2,70% (n = 2,912) y con edades entre 29 y 39 años, con 54,9% (n = 2,024) viviendo en el interior del estado y un total de 0,91% (n = 1247) con escuela primaria incompleta. El pre-cribado clínico, de laboratorio y serológico del virus de la hepatitis B en candidatos a donación de sangre es un protocolo de seguridad para el procedimiento de transfusión. Con el objetivo de incrementar la reducción de los riesgos de contaminación y pérdida de insumos utilizados para la donación, es necesario movilizar campañas de vacunación contra el virus de la hepatitis B, para todos los que soliciten donar sangre en los Centros de Sangre.No decorrer dos últimos anos vários avanços ocorreram na medicina transfusional, assim como o desenvolvimento de técnicas laboratoriais utilizadas na triagem sorológica de doadores de sangue, objetivando minimizar a transmissão de doenças infecciosas. A importância da detecção do anti-HBc total objetiva auxiliar no diagnóstico diferencial de hepatites, acompanhando a infecção pelo vírus da hepatite B (HBV) em conjunto com outros marcadores virais, sendo utilizado em bancos de sangue como teste de triagem para doadores de sangue, apresentando eficácia na detecção ao contato prévio com o HBV durante a “janela negativa” do HBV. O objetivo desse estudo foi avaliar o perfil sorológico para hepatite B (anti-HBc total) em candidatos à doação de sangue em uma região do nordeste do Brasil. Foi realizado um estudo transversal, de abordagem qualitativa, com o sistema de informatização de todo o ciclo de doação de sangue, com dados retrospectivos e categorizados de acordo com sexo, faixa etária, localização, grau de instrução e comportamento de risco, tendo como população-alvo candidatos à doação de sangue no, no período de janeiro de 2016 a dezembro de 2020. Dos 136.652 candidatos à doação, foi observada reatividade ao anti-HBc total em 2,69% (N=3.681). Ocorreu maior reatividade ao anti-HBc total entre os candidatos do sexo masculino 2,70% (n=2.912) e faixa etária entre 29 a 39 anos, sendo que 54,9% (n=2.024) residem no interior do estado e um total de 0,91% (n=1.247) com o 1° grau incompleto. A pré-triagem clínica, laboratorial e sorológica para o vírus da hepatite B, em candidatos à doação de sangue, é um protocolo de segurança para o ato transfusional.
