More Doctors Program and the tendency of hospitalizations for Ambulatory Care sensitive conditions in Sergipe, Brazil, from 2014 to 2019

Resumo: The study aimed to analyze the evolution of the rates of hospitalizations for Ambulatory Care Sensitive Conditions (ACSC) in Sergipe and its municipalities between 2014 to 2019, correlating these admissions with the percentage of doctors from the More Doctors Program (MDP) in the Family Health Strategy’s (FHS) teams and correlating the participation in the program with the Human Development Index (HDI) of the municipalities. The results obtained showed that, after dividing the municipalities in the state into groups according to the percentage of physicians by the MDP in the FHS teams, a downward trend was observed in the admissions for ACSC in all 3 groups formed, but the dimension of this drop was greater in the group with the highest percentage of physicians in the More Doctors Program. In addition, using Spearman's correlation, there was a negative correlation in 2014 and 2015 between Sergipe’s municipalities with the lowest HDI and a higher percentage of physicians by More Doctors in the FHS teams, and a positive correlation from 2016 to 2019 between municipalities with higher HDI and higher rates of hospitalizations for ACSC. These findings suggest that the MDP may have influenced this reduction in hospitalization rates, but that this assessment deserves to be deepened by further studies. It was also verified little literature about the MDP in Sergipe, which demonstrates the need for this program to be better evaluated through other variables.

Los resultados obtenidos mostraron que, después de dividir los municipios del estado en grupos según el porcentaje de médicos por el PMM en los equipos de la ESF, se observó una tendencia a reducción de los ingresos por ACSC en todos los 3 grupos formados, pero la dimensión de este descenso fue mayor en el grupo con mayor porcentaje de médicos en el programa. Además, utilizando la correlación de Spearman, hubo una correlación negativa en 2014 y 2015 entre los municipios de Sergipe con el IDH más bajo y un porcentaje más alto de médicos por el PMM en los equipos de la ESF, y una correlación positiva de 2016 a 2019 entre los municipios con IDH más alto y tasas de hospitalizaciones por ACSC más altas. Estos resultados sugieren que el PMM puede haber influido en esta reducción de las tasas de hospitalización, pero que esta evaluación merece ser profundizada en más estudios. También hubo poca literatura sobre el PMM en Sergipe, lo que demostró la necesidad de que este programa sea mejor evaluado mediante otras variables.O estudo objetivou analisar a evolução das taxas de Internações por Condições Sensíveis à Atenção Primária em Sergipe e seus municípios no período de 2014 a 2019 correlacionando essas internações com o percentual de médicos pelo Programa Mais Médicos nas equipes da Estratégia de Saúde da Família e correlacionando a participação no programa com o Índice de Desenvolvimento Humano dos municípios. Os resultados obtidos demonstraram que, após a divisão das municipalidades do estado em grupos de acordo com o percentual de médicos pelo PMM nas equipes da ESF, observou-se tendência de queda nas taxas de ICSAP nos 3 grupos formados e a dimensão dessa queda foi maior no grupo de maior percentual de médicos pelo programa. Esses achados sugerem que o PMM pode ter influenciado nessa redução das taxas de internações, mas que essa avaliação merece ser aprofundada por estudos posteriores.
