The use of dexmedetomidine in agitation in children: a literature review

Resumo: | Introduction: The incidence of postoperative agitation in children under general anesthesia is high, and can be manifested by several behaviors, such as crying, irritability, intense agitation and disorientation, which in many cases require mechanical restraint. The use of dexmedetomidine in this situation is well described by several authors. This sedative provides a “conscious sedation” to the patient, where they appear to be asleep but can be easily aroused without respiratory depression. Objectives: To review the literature on the use of dexmedetomidine to control agitation in children undergoing operative anesthesia, in addition to comparing the use of other sedatives, such as ketamine, with dexmedetomidine. Methodology: Narrative bibliographic review, through data collection, using the Scielo, MedLine and PubMed database platforms, with the descriptors: “dexmedetomidine” AND “psychomotor agitation” AND “child” OR “clonidine” AND "ketamine". Results and Conclusion: Most studies have shown that dexmedetomidine is well applied to agitation in children, promoting postoperative pain relief, with a low incidence of adverse events, and thus can be used safely. In addition, other drugs, such as ketamine, have also shown satisfactory results for the prevention of postoperative agitation in children. |

El uso de dexmedetomidina en esta situación está bien descrito por varios autores. Este sedante proporciona una "sedación consciente" al paciente, donde parece estar dormido pero puede despertarse fácilmente sin depresión respiratoria. Objetivos: Revisar la literatura sobre el uso de dexmedetomidina para controlar la agitación en niños sometidos a anestesia operatoria, además de comparar el uso de otros sedantes, como la ketamina, con la dexmedetomidina. Metodología: Revisión bibliográfica narrativa, mediante recolección de datos, utilizando las plataformas de base de datos Scielo, MedLine y PubMed, con los descriptores: “dexmedetomidina” Y “psicomotora agitación” Y “niño” O “clonidina” Y “ketamina”. Resultados y Conclusión: La mayoría de los estudios han demostrado que la dexmedetomidina se aplica bien a la agitación en los niños, promoviendo el alivio del dolor postoperatorio, con una baja incidencia de eventos adversos y, por lo tanto, puede usarse de manera segura. Además, otros fármacos, como la ketamina, también han mostrado resultados satisfactorios para la prevención de la agitación postoperatoria en niños.Introdução: A incidência de agitação pós-operatória em crianças sob anestesia geral é alta, e pode ser manifestada por diversos comportamentos, como choro, irritabilidade, intensa agitação e desorientação, que em muitos casos, necessita de contenção mecânica. O uso de dexmedetomidina nessa situação é bem descrito por vários autores. Este sedativo fornece uma “sedação consciente” ao paciente, onde parecem adormecidos, mas podem ser facilmente despertáveis sem depressão respiratória. Objetivos: Realizar uma revisão da literatura sobre o uso da dexmedetomidina no controle de agitação em crianças submetidas a anestesia operatória, além de comparar o uso de outros sedativos, como cetamina, com a dexmedetomidina. Metodologia: Revisão bibliográfica narrativa, por meio do levantamento de dados, utilizando as plataformas de base de dados Scielo, MedLine e PubMed, com os descritores: “dexmedetomidine” AND “psychomotor agitation” AND “child” OR “clonidine” AND “ketamine”. Resultados e Conclusão: A maior parte dos estudos mostrou que a dexmedetomidina é bem aplicada na agitação em crianças, promovendo alívio de dor pós-operatória, com baixa incidência de eventos adversos, podendo, assim, ser utilizada com segurança.
