Analysis of consumption and spending with potentially hazardous medicines in a tertiary hospital of the northeast Brazil

Resumo: The ABC curve has been widely indicated in order to achieve efficient management of medicines in stock, with the Potentially Hazardous Medicines (MPP's) group being responsible for a considerable demand within the hospital. This study aims to analyze the consumption and expenditure on standardized MPP's in a tertiary hospital in Northeastern Brazil using the ABC classification method. This is a descriptive and retrospective study, with quantitative and qualitative approach based on the analysis of data obtained from drugs transferred from the Pharmaceutical Supply Center (CAF), to the destination stock, Central Pharmacy, from March 2019 to August 2019. Items obtained after collection were categorized and classified according to the ABC curve. Data analysis was performed using descriptive statistics: frequency and percentage. The results showed that 11.50% of the items were categorized in class A and comprised 81.88% of the value, 15.40% of the items were classified in class B representing 12.76% of the expenses and 73.10% represented only 5.36% of the total cost, being listed in class C. The drugs that were in position A of the curve were: Heparin 5,000 IU / ml (0.25ml), Parenteral Nutrition Solutions (1000ml bag) and Enoxaparin 40mg / 0.4ml, representing respectively 30.25%, 28.36% and 23.18% of the percentage of the total amount spent. From the results obtained, it can be seen that MPP's generate significant financial impacts to hospital institutions, especially in the case of antithrombotic agents such as heparin and enoxaparin. The ABC classification therefore allows the priority items to be highlighted for cost management measures.

Este estudio tiene como objetivo analizar el consumo y el gasto en MPP estandarizados en un hospital terciario en el noreste de Brasil utilizando el método de clasificación ABC. Este es un estudio descriptivo y retrospectivo, con enfoque cuantitativo y cualitativo basado en el análisis de datos obtenidos de medicamentos transferidos desde el Centro de Abastecimiento Farmacéutico (CAF), al stock de destino, Farmacia Central, desde marzo de 2019 hasta Agosto de 2019. Los artículos obtenidos después de la recolección fueron categorizados y clasificados de acuerdo con la curva ABC. El análisis de los datos se realizó mediante estadística descriptiva: frecuencia y porcentaje. Los resultados mostraron que el 11.50% de los artículos se clasificaron en la clase A y comprendieron el 81.88% del valor, el 15.40% de los artículos se clasificaron en la clase B que representa el 12.76% de los gastos y el 73.10% representaron solo el 5.36% del costo total, que figuran en la clase C. Los medicamentos que estaban en la posición A de la curva fueron: heparina 5,000 UI / ml (0.25 ml), soluciones de nutrición parenteral (bolsa de 1000 ml) y enoxaparina 40 mg / 0.4ml, que representan respectivamente el 30.25%, 28.36% y 23.18% del porcentaje del monto total gastado. De los resultados obtenidos, se puede ver que las MPP generan impactos financieros significativos en las instituciones hospitalarias, especialmente en el caso de los agentes antitrombóticos como la heparina y la enoxaparina. Por lo tanto, la clasificación ABC permite resaltar los elementos prioritarios para las medidas de gestión de costos.A curva ABC tem sido muito indicada com o intuito de obter-se uma gestão eficiente de medicamentos em estoque, sendo o grupo dos Medicamentos Potencialmente Perigosos (MPP’s) responsáveis por uma considerável demanda no âmbito hospitalar. Os resultados apontaram que 11,50% dos itens, foram categorizados na classe A e compreenderam 81,88% do valor, 15,40% dos itens classificaram-se na classe B representando 12,76% dos gastos e 73,10% representaram apenas 5,36% do custo total, sendo listados na classe C. Os medicamentos que se encontraram na posição A da curva foram: Heparina 5.000UI/ml (0,25ml), Soluções de Nutrição Parenteral (bolsa 1000ml) e Enoxaparina 40mg/0,4ml, representando respectivamente 30,25%, 28,36% e 23,18% do percentual do valor total gasto. A partir dos resultados obtidos, pode-se perceber que os MPP’s geram impactos financeiros significativos às instituições hospitalares, especialmente em se tratando de agentes antitrombóticos, como a heparina e enoxaparina.
