Paracoccidioidomycosis in an immunocompetent adult South of Minas Gerais: Case report

Resumo: Paracoccidioidomycosis (PCM) is an infection that develops after inhalation of fungal parts dispersed in the atmosphere. It affects mainly males living in rural areas and since there is a higher incidence of the fungus in this environment. It presents several clinical manifestations, thus making the outcome of the diagnosis difficult. In this report, we describe the case of a 59-year-old man with a history of syncope followed by several generalized tonic-clonic seizures. After hospitalization and several complementary exams, the investigation culminated in the anatomopathological analysis of the transrectal resection of the prostate, investigating the presence of fungus, which resulted positive by PAS and Gragott staining for Paracoccidioides brasiliensis. The patient underwent treatment with Sulfamethoxazole 400 mg +Trimethoprim 80 mg, for two years, until clinical improvement and the results of requested complementary exams were obtained. The patient is under monitoring and has had no recurrences of the disease so far.

Afecta principalmente a varones residentes en las zonas rurales, pues hay una mayor incidencia de esporas fúngicas suspendidas en el aire en éstas zonas. Puede presentar diversas manifestaciones clínicas, lo que dificulta el diagnóstico. El presente reporte describe el caso de un varón de 59 años con un historial médico de síncope seguido de convulsiones tónico clónico generalizado. Después de múltiples análisis la investigación siguió hasta realizar una resección transrectal de la próstata en la búsqueda por hongos. El resultado fue positivo para PAS y tición de Gragott de Paracoccidioides brasiliensis. El paciente fue sometido al tratamiento con Sulfametoxazol 400 mg + Trimetoprima 80 mg a lo largo de dos años, hasta observar mejoría clínica y mejoría en los resultados de las pruebas complementarias.A Paracoccidioidomicose (PCM), é uma infecção instaurada após a inalação de partes fúngicas, dispersas na atmosfera. Acomete principalmente o sexo masculino e moradores da zona rural, visto que há maior incidência do fungo nesse meio. Apresenta diversas manifestações clínicas, dificultando assim o desfecho do diagnóstico. Nesse relato em questão, é descrito o caso de um senhor de 59 anos com histórico de síncope, seguido de diversas convulsões do tipo tônico-clônica generalizadas. Após internação e com diversos exames complementares, a investigação culminou na análise anatomopatológica da ressecção transretal da próstata, investigando a presença de fungos, na qual resultou positiva pela coloração de PAS e Gragott da Paracoccidioides brasiliensis. O paciente foi submetido ao tratamento com Sulfametoxazol 400 mg +Trimetoprima 80 mg, por dois anos, até que se obteve uma melhora clínica e dos resultados de exames complementares solicitados.
