Reporting of adverse events related to pediatric medication errors: integrative review

Resumo: | Objectives: Identify whether health professionals report adverse events related to medication errors and analyze the factors that lead them not to report adverse events, in addition to discussing the main medication errors commited by health professionals. Methodology: This is an integrative, descriptive and qualitative literature review, which used the PICO strategy to elaborate the following question: Do health professionals report adverse events related to medication errors in pediatrics and what factors lead them not to do so? Data collection was carried out between December 2021 and March 2022. The following databases were used: LILACS, MEDLINE, Cochrane Library and Scopus®. The descriptors used were: (“medication errors”), (“child”), (“pediatrics”) and (“notice”), using the boolean operators AND and OR. Results: 10 articles were selected that discuss the notifications of adverse events related to medicines in pediatrics and it was observed that most of the production was concentrated abroad, especially in Europe. Half of the studies performed and/or proposed interventions to improve practices for reporting adverse drug events. However, many surveys used surveillance and/or reporting pharmacovigilance systems from their country to carry out the same studies (40%). Conclusion: Strategies on medication errors aim to promote and disseminate patient safety as well as minimize the occurrence of errors. It should be based on increasing the level of staff confidence, greater clarity and training on reporting systems. |

Metodología: Se trata de una revisión integradora de la literatura, descriptiva y cualitativa, que utilizó la estrategia en la elaboración de la pregunta: ¿Los profesionales de la salud notifican los eventos adversos relacionados con errores de medicación en pediatría y qué factores los llevan a no hacerlo? La recogida de datos se realizó entre diciembre de 2021 y marzo de 2022. Se utilizaron las siguientes bases de datos: LILACS, MEDLINE, Cochrane Library y Scopus®. Se utilizaron los descriptores: (“medication errors”), (“child”), (“pediatrics”) y (“notice”), utilizando los operadores booleanos AND y OR. Resultados: Se seleccionaron 10 artículos que discuten las notificaciones de eventos adversos relacionados con medicamentos en pediatría y se observó que la mayor parte de la producción se concentró en el exterior, especialmente en Europa. La mitad de los estudios realizaron y/o propusieron intervenciones para mejorar las prácticas de notificación de eventos adversos relacionados con los medicamentos. Sin embargo, muchas investigaciones utilizaron los sistemas de vigilancia y/o de notificación de farmacovigilancia de su país para llevar a cabo lo mismo (40%). Conclusión: Las estrategias sobre errores de medicación tienen como objetivo promover y difundir la seguridad del paciente, así como minimizar la ocurrencia de errores. Debe basarse en aumentar el nivel de confianza del equipo, en una mayor claridad y capacitación sobre los sistemas de notificación.Objetivos: Identificar se há a realização de notificação de eventos adversos relacionados ao erro de medicação pelos profissionais de saúde e analisar os fatores que os levam a não realizarem as notificações dos eventos adversos, além de, discutir sobre os principais erros de medicação acometidos pelos profissionais de saúde. Metodologia: Trata-se de uma revisão integrativa da literatura, descritiva e qualitativa, que utilizou a estratégia PICO na confecção da questão: Os profissionais de saúde realizam a notificação de eventos adversos relacionados ao erro de medicação em pediatria e quais fatores os levam a não realizarem? A coleta de dados foi realizada entre dezembro de 2021 a março de 2022. Foram utilizadas as bases de dados: LILACS, MEDLINE, Cochrane Library e Scopus®. Foram utilizados os descritores: (“medication erros”), (“child”), (“pediatrics”) e (“notice”), dispondo-se os operadores booleanos AND e OR. Resultados: Foram selecionados 10 artigos que discutem sobre as notificações de eventos adversos relacionados a medicamentos em pediatria e observou-se que a maior parte da produção concentrava-se no exterior, com destaque na Europa. Metade estudos, realizaram e/ou propuseram intervenções para melhorias nas práticas de notificações de eventos adversos relacionados a medicamentos. Contudo, muitas pesquisas utilizaram sistemas de vigilância e/ou farmacovigilância de notificação do seu país para realização do mesmo (40%). Conclusão: As estratégias sobre os erros de medicação visam promover e difundir a segurança do paciente bem como minimizar a ocorrência de erros.
