Effect of laser phototherapy on algiic perception, fear of movement and quality of life of subjects with myofascial pain associated with fibromyalgia

Resumo: | The study evaluated the effectiveness of laser phototherapy on the myofascial trigger point of the trapezius muscle in fibromyalgic patients. 25 individuals were diagnosed with myofascial pain and fibromyalgia and divided into groups: Laser (n=8), Laser + medication (n=10) and Medication (n=7) and treated for 5 weeks. The variables measured were: 1) Pain intensity using the Visual Analogue Scale (VAS); 2) Pressure pain threshold (PPT) using a pressure algometer; 3) General pain status using the Rolland-Morris Questionnaire (QIRM); 4) Fear of moving according to the Tampa Scale for Kinesiophobia (ETC) and 5) Assessment of the impact of Fibromyalgia on quality of life (QIF). The data obtained were analyzed using two-way analysis of variance (ANOVA) for repeated measures, followed by Tukey's post-test, when appropriate. For cases where the interaction between the factors (Treatment and Time) was significant, one-way repeated measures ANOVA was conducted for each treatment group. All tests were two-tailed and p<0.05 was considered. There was a decrease in pain intensity and an increase in PPT in the upper trapezius muscle in the 5th and 10th sessions compared to baseline for the groups in which phototherapy was used. There was no significant interaction between the scores of the IRM, ETC and IQF variables between the groups. The QIF scale, rigidity and anxiety, showed a significant difference between the groups. Laser phototherapy promoted a decrease in local pain intensity and an increase in trapezius muscle PPT. |

Las variables medidas fueron: 1) Intensidad del dolor mediante la Escala Visual Analógica (EVA); 2) Umbral de dolor a la presión (PPT) utilizando un algómetro de presión; 3) Estado general del dolor mediante el Cuestionario de Rolland-Morris (QIRM); 4) Miedo a moverse según la Escala de Tampa para Kinesiofobia (ETC) y 5) Evaluación del impacto de la Fibromialgia en la calidad de vida (QIF). Los datos obtenidos se analizaron mediante análisis de varianza de dos vías (ANOVA) para medidas repetidas, seguido de la prueba posterior de Tukey, cuando correspondía. Para los casos en los que la interacción entre los factores (Tratamiento y Tiempo) fue significativa, se realizó ANOVA de medidas repetidas unidireccionales para cada grupo de tratamiento. Todas las pruebas fueron de dos colas y se consideró p<0,05. Hubo una disminución en la intensidad del dolor y un aumento en el PPT en el músculo trapecio superior en las sesiones 5 y 10 en comparación con la línea de base para los grupos en los que se utilizó fototerapia. No hubo interacción significativa entre los puntajes de las variables IRM, ETC y IQ entre los grupos. La escala QIF, rigidez y ansiedad, mostró diferencia significativa entre los grupos. As variáveis mensuradas foram: 1) Intensidade da dor pela Escala Visual Analógica (EVA); 2) Limiar de dor à pressão (LDP) por meio de algômetro de pressão; 3) Estado geral da dor por meio do Questionário de Rolland-Morris (QIRM); 4) Medo de movimentar pela Escala de Tampa para Cinesiofobia (ETC) e 5) Avaliação do impacto da Fibromialgia na qualidade de vida (QIF). Os dados obtidos foram analisados por meio da análise de variância (ANOVA) de duas vias para medidas repetidas, seguida pelo pós-teste de Tukey, quando apropriado. Para os casos em que a interação entre os fatores (Tratamento e Tempo) foi significativa, foi conduzida ANOVA para medidas repetidas de uma via para cada grupo de tratamento. Todos os testes foram bi-caudais e foi considerado p<0,05. Verificou-se diminuição da intensidade da dor e aumento do LDP no músculo trapézio superior na 5ª e 10ª sessões em relação ao basal para os grupos em que a fototerapia foi utilizada. Não houve interação significativa entre os escores das variáveis QIRM, ETC e QIF entre os grupos. A escala QIF, rigidez e ansiedade, mostraram diferença significativa entre os grupos.
