Oxyhemodynamic effectiveness of three types of bed baths for patients with severe heart disease: crossover

Resumo: | Objective: to identify hemodynamic and/or oximetric changes resulting from three bed bath techniques in cardiac patients admitted to a Surgical Intensive Care Unit. Method: 3 X 3 crossover clinical trial in which three bed bath technologies were applied to the same individual, at three different times. The interventions considered were: traditional bath at 42.5ºC with monitored and constant water temperature; Bath with 1L vial of distilled water heated for 1 minute in the microwave oven at full power without temperature control; Bath with wet wipes heated for 20 seconds in the microwave oven at full power. Sample comprised of 18 patients in the postoperative period of cardiac surgery. The variables heart rate, respiratory rate, blood pressure (systolic, diastolic and mean), axillary temperature and oxygen saturation using pulse oximetry were analyzed. To compare the means, the ANOVA Two-way test of repeated measures with a pre-established significance level of 5% was used. Results: Sample predominantly composed by male, brown and non-smoker individuals, who underwent coronary artery bypass surgery. There was no significant change in the variables studied (p > 0.05), considering the different types of bath at different times (before, during and after the procedure). Conclusion: Bed bath with water at 42.5 ° C, with heated distilled water vial and wet wipes, in post-cardiac surgery patients admitted to the ICU, does not cause any significant changes in the oxy-hemodynamic variables. |

Método: Ensayo clínico Crossover 3 X 3 en el que se aplicaron tres tecnologías de baño de lecho, en el mismo individuo, en tres momentos diversos. Las intervenciones consideradas fueron: baño tradicional a 42.5ºC con temperatura del agua monitoreada y constante; Baño con un vial de 1L de agua destilada calentada durante 1 minuto en el horno de microondas a plena potencia sin control de temperatura; Baño con toallitas húmedas calentadas durante 20 segundos en el horno de microondas a plena potencia. Muestra compuesta por 18 pacientes en el postoperatorio de cirugía cardíaca. Las variables frecuencia cardíaca, frecuencia respiratoria, presión arterial (sistólica, diastólica y media), temperatura axilar y saturación de oxígeno mediante oximetría de pulso fueron analizadas. Para comparar las medias, se utilizó la prueba de dos vías ANOVA de medidas repetidas con un nivel de significancia preestablecido del 5%. Resultados: muestra predominantemente masculina, morena y no fumadora, que se sometió a cirugía de revascularización coronaria. No hubo cambios significativos en las variables estudiadas (p > 0.05), considerando los diferentes tipos de baño en diferentes momentos (antes, durante y después del procedimiento). Conclusión: Los baños de lecho con agua a 42.5°C, con vial de agua destilada calentada y toallitas húmedas, en pacientes después de una cirugía cardíaca y admitidos en la UCI, no causan cambios significativos en las variables oxi-hemodinámicas.Objetivo: identificar alterações hemodinâmicas e/ou oximétricas decorrentes de três técnicas de banho no leito em pacientes cardiopatas internados em uma Unidade de Terapia Intensiva Cirúrgica. Método: Ensaio clínico tipo crossover 3 X 3 no qual foram aplicadas três tecnologias de banho no leito, no mesmo indivíduo, em três momentos diferentes. As intervenções consideradas foram: banho tradicional a 42,5ºC com temperatura da água monitorada e constante; Banho com frasco-ampola de 1L de água destilada aquecido por 1 minuto no forno de micro-ondas em potência máxima sem controle da temperatura; Banho com lenços umedecidos aquecidos por 20 segundos no forno de micro-ondas em potência máxima. Amostra composta por 18 pacientes em pós-operatório de cirurgias cardíacas. Foram analisadas as variáveis frequência cardíaca, frequência respiratória, Pressão arterial (sistólica, diastólica e média), temperatura axilar e saturação de oxigênio por meio de oximetria de pulso. Para comparação das médias, foi utilizado o teste ANOVA Two-way de medidas repetidas com nível de significância pré-estabelecido de 5%. Resultados: Amostra predominantemente masculina, parda e não fumante, submetida majoritariamente à cirurgia de revascularização do miocárdio. Não houve nenhuma alteração significativa nas variáveis estudadas (p > 0,05), considerando os diversos tipos de banho nos diferentes momentos (antes, durante e após o procedimento).
