Evaluation of the prescription of antibiotics by endodontists in acute periapical abscess

Resumo: This study aimed to evaluate the prescription of antibiotics in acute periapical abscesses by endodontists in the metropolitan region of Recife - PE. It was a cross-sectional, quantitative and descriptive study, with a sample of 100 endodontists, who worked in dentistry schools and private or public dental clinics. They answered a questionnaire on the prescription of systemic antibiotics in the treatment of acute periapical abscess and the data were analyzed descriptively using absolute and relative frequencies. Pearson's chi-square test or Fisher's exact test was used to evaluate the association between two categorical variables and the statistical program used was SPSS (Statistical Package for the Social Sciences) in version 23. The association of amoxicillin with potassium clavulanate in adult patients without penicillin allergy was prescribed by 50% of endodontists and in cases of penicillin allergy, clindamycin was the drug of choice with 67%. Most endodontists prescribed the antibiotic for 7 days. Regarding the prescription of antibiotics, most responded that they did not prescribe in all phases of the acute periapical abscess, however, when the evolutionary stages of this disease were determined, the results showed that there was a prescription in all phases, but with different percentages, being indicated when there was systemic involvement of the patient. It was concluded that the prescription of antibiotics by the endodontists surveyed was not consistent with the scientific literature, however, it showed a concern with the health and risk of serious systemic complications of the patient.

Un estudio transversal con una muestra de 100 endodoncistas, quienes realizaron sus actividades en escuelas dentales y clínicas dentales privadas o públicas. Respondieron un cuestionario sobre la prescripción de antibióticos sistémicos en el tratamiento del absceso periapical agudo y los datos se analizaron de forma descriptiva utilizando frecuencias absolutas y relativas. Se utilizó la prueba de chi-cuadrado de Pearson o la prueba exacta de Fisher para evaluar la asociación entre dos variables categóricas y el programa estadístico utilizado fue SPSS (Statistical Package for the Social Sciences) en la versión 23. La asociación de amoxicilina con clavulanato de El potasio en pacientes adultos sin alergia a la penicilina fue prescrito por el 50% de los endodoncistas y en los casos de alergia a la penicilina, la clindamicina fue el fármaco de elección con el 67%. La mayoría de los endodoncistas prescribieron el antibiótico durante 7 días. En cuanto a la prescripción de antibióticos, la mayoría respondió que no prescribían en todas las fases del absceso periapical agudo, sin embargo, cuando se determinaron las etapas evolutivas de esta enfermedad, los resultados mostraron que hubo prescripción en todas las fases, pero con diferentes porcentajes, siendo más indicado cuando hubo afectación sistémica del paciente. Se concluyó que la prescripción de antibióticos por parte de los endodoncistas encuestados no fue consistente con la literatura científica, sin embargo, mostró preocupación con la salud y riesgo de complicaciones sistémicas graves del paciente.Esse estudo teve como objetivo avaliar a prescrição de antibióticos nos abscessos periapicais agudos pelos endodontistas da região metropolitana do Recife - PE. Um estudo transversal com uma amostra de 100 endodontistas, que exerciam suas atividades em faculdades de odontologia e clínicas odontológicas particulares ou públicas. Eles responderam um questionário sobre prescrição de antibióticos sistêmicos no tratamento de abscesso periapical agudo e os dados foram analisados descritivamente por meio de frequências absolutas e relativas. Para avaliar associação entre duas variáveis categóricas foi utilizado o teste Qui-quadrado de Pearson ou teste Exato de Fisher e o programa estatístico utilizado foi o SPSS (Statistical Package for the Social Sciences) na versão 23. A associação de amoxicilina com clavulanato de potássio em pacientes adultos sem alergia a penicilina foi prescrita por 50% dos endodontistas e em casos de alergia a penicilina, a clindamicina foi a droga de primeira escolha com 67%. A maioria dos endodontistas realizava a prescrição do antibiótico por 7 dias. Com relação a prescrição de antibióticos a maioria respondeu que não prescrevia em todas as fases do abscesso periapical agudo, no entanto, quando houve determinação das fases evolutivas dessa doença, resultados mostraram que existiu prescrição em todas as fases, porém com percentuais diferentes, sendo mais indicados quando existia comprometimento sistêmico do paciente.
