Ocular hypertension in pediatrics patients with acute lymphoblastic leukemia

Resumo: Objectives: To evaluate the frequency and associated factors with ocular hypertension (OH) in patients with acute lymphoblastic leukemia (ALL) using glucocorticoids (GC), in order to determine intraocular pressure (IOP) at four times along the treatment (first day – D0, after eight days – D8, after twenty-eight days – D28 and after 6 months – D6m) and to identify association of OH with IOP, age group, cerebrospinal fluid (CSF) infiltration, immunophenotyping (IMP), sex and death. Methodology: Prospective descriptive study design enrolling children with ALL under chemotherapy at a public and specialized in pediatric oncology ward from January/2013 until December/2017. IOP was measured on D0, D8, D28 and D6m of treatment. Measurements above 21 mmHg were considered HO. Results: Of the 58 patients included (aged between 1.4 and 19.2 years), 17.2% (10 patients) had OH, with higher blood pressure increases on D8 (p<0.001), and the two more cases on D28. No patient had symptoms and the pressures normalized after cessation of GC on D6m. The variables sex, age, IMP and CSF infiltration were not a risk for OH. It was not possible to predict mortality and CSF infiltration related to IOP values. Conclusion: Data analysis identified 17.2% of HO in the sample, the highest IOP means were concentrated on D8 and two other cases occurred on D28. Pre-treatment clinical and laboratory features were not associated with IOP elevation or CSF blast infiltration and death.

Metodología: Diseño de estudio descriptivo y prospectivo que inscribió a niños con LLA en quimioterapia en un servicio público y especializado en oncología pediátrica desde enero/2013 hasta diciembre/2017. La PIO se midió en D0, D8, D28 y D6m de tratamiento. Las medidas superiores a 21 mmHg se consideraron HO. Resultados: De los 58 pacientes incluidos (edad entre 1,4 y 19,2 años), 17,2% (10 pacientes) presentaban HO, con mayores aumentos de presión arterial el D8 (p<0,001), y los dos casos más el D28. Ningún paciente presentó síntomas y las presiones se normalizaron tras el cese de GC en D6m. Las variables sexo, edad, IMF e infiltración de LCR no fueron de riesgo para HO. No fue posible predecir mortalidad e infiltración de LCR en relación con los valores de PIO. Conclusión: El análisis de datos identificó 17,2% de HO en la muestra, las medias de PIO más altas se concentraron en D8 y otros dos casos ocurrieron en D28. Las características clínicas y de laboratorio previas al tratamiento no se asociaron con elevación de la PIO o infiltración por blastos de LCR y muerte.Objetivos: Avaliar a frequência e fatores associados à hipertensão ocular (HO) em pacientes com leucemia linfoide aguda (LLA) em uso de glicocorticoides (GC), determinar a pressão intraocular (PIO) em quatro momentos do tratamento (primeiro dia – D0, após oito dias – D8, após vinte e oito dias – D28 e após 6 meses – D6m) e identificar associação da HO com PIO, faixa etária, infiltração do líquido cefalorraquidiano (LCR), imunofenotipagem (IMF), sexo e óbito. Metodologia: Estudo prospectivo descritivo em crianças com LLA em quimioterapia de um serviço público especializado em oncologia pediátrica entre janeiro/2013 e dezembro/2017. A PIO foi aferida em D0, D8, D28 e D6m de tratamento. Medidas acima de 21 mmHg foram considerados HO. Resultados: Dos 58 pacientes incluídos (idade entre 1,4 e 19,2 anos), 17,2% (10 pacientes) apresentaram HO, sendo os aumentos pressóricos maiores no D8 (p<0,001), com dois casos no D28. Nenhum paciente apresentou sintomas e as pressões normalizaram após a cessação do GC no D6m. As variáveis sexo, idade, IMF e infiltração do LCR não se comportaram como risco para HO. Não foi possível predizer mortalidade e infiltração do LCR relacionados aos valores da PIO. Conclusão: A análise dos dados identificou 17,2% de HO na amostra, as maiores médias da PIO se concentraram no D8 e outros dois casos ocorreram no D28.
