Evaluation of vaccine rooms: a Brazilian case study

Resumo: The study aims to evaluate the situation of vaccine rooms in a Brazilian city based on the standard required by the Ministry of Health, in Primary Health Care. This is an evaluative research, with a qualitative approach, carried out in Campina Grande-PB. Interviews were carried out with 15 nurses, technicians responsible for the vaccine rooms, with a script based on the Instrument for Supervision of Vaccination Rooms of the National Immunization Program, considered the gold standard in the evaluation of this service. Data were submitted to Content Analysis. The standard was not reached in several items evaluated, with the local reality permeated by particular situations that interfere with the quality of services: lack of immunobiological and vaccinators, culminating in vaccine delay; professionals' lack of knowledge about the reasons for vaccine contraindications and about the notification of vaccine-prevented diseases in the coverage area; lack of standard operating procedure for carrying out the activities inherent to the vaccination process. The vaccine rooms in the studied municipality are in disagreement with ministerial norms, which makes it difficult to implement good vaccine practices, in this context, the fragility of the municipality's responsibility regarding the planning and organization of vaccine rooms is perceived. Thus, there is a need for investment to improve the physical structure of the rooms and equipment; to promote the training and continuing education of professionals to work in the vaccine room.

Se trata de una investigación evaluativa, con abordaje cualitativo, realizada en Campina Grande-PB. Se realizaron entrevistas a 15 enfermeros, técnicos responsables de las salas de vacunación, con un guión basado en el Instrumento de Supervisión de Salas de Vacunación del Programa Nacional de Inmunizaciones, considerado el estándar de oro en la evaluación de este servicio. Los datos se enviaron al análisis de contenido. El estándar no fue alcanzado en varios ítems evaluados, con la realidad local permeada por situaciones particulares que interfieren en la calidad de los servicios: falta de inmunobiológicos y vacunadores, culminando en el retraso de las vacunas; falta de conocimiento de los profesionales sobre las razones de las contraindicaciones de las vacunas y sobre la notificación de las enfermedades prevenibles por vacunación en el área de cobertura; falta de un procedimiento operativo estándar para la realización de las actividades inherentes al proceso de vacunación. Las salas de vacunas del municipio estudiado están en desacuerdo con las normas ministeriales, lo que dificulta la implementación de buenas prácticas vacunales, en ese contexto, se percibe la fragilidad de la responsabilidad del municipio en cuanto a la planificación y organización de las salas de vacunas. Por lo tanto, existe la necesidad de inversión para mejorar la estructura física de las salas y el equipamiento; promover la formación y educación continua de los profesionales para actuar en la sala de vacunas.O estudo objetiva avaliar a situação das salas de vacinas em um município brasileiro a partir do padrão exigido pelo Ministério da Saúde, na Atenção Primária à Saúde. Trata- se de uma pesquisa avaliativa, com abordagem qualitativa, realizada em Campina Grande-PB. Realizou-se entrevistas com 15 enfermeiros, responsáveis técnicos pelas salas de vacina, com roteiro formulado a partir do Instrumento de Supervisão das Salas de Vacinas do Programa Nacional de Imunizações, considerado padrão-ouro na avaliação desse serviço. Os dados foram submetidos à Análise de Conteúdo. O padrão não foi alcançado em diversos itens avaliados, sendo a realidade local permeada de situações particulares que interferem na qualidade dos serviços: falta de imunobiológico e vacinadores, culminando em atraso vacinal; desconhecimento dos profissionais sobre os motivos de contraindicações de vacinas e acerca da notificação de doenças prevenidas por vacina na área de abrangência; inexistência de procedimento operacional padrão para realização das atividades inerentes ao processo de vacinação. As salas de vacinas do município estudado estão em desacordo com as normas ministeriais, o que dificulta a execução das boas práticas de vacina, neste contexto se percebe a fragilidade da responsabilidade do município quanto ao planejamento e organização das salas de vacinas.
