Botulinum toxin in facial aesthetics and the processing of emoticons

Resumo: The action of botulinum toxins is the result of their powerful ability to inhibit the release of neurotransmitters. In this sense, botulinum toxins have a wide range of indications, both cosmetic and therapeutic. The aim of this study was to carry out a bibliographical survey on the use of botulinum toxin Type A (BoNT/A) in facial aesthetics and emotion processing. To this end, we evaluated randomized clinical trials (RCTs) that analyzed the effects of BoNT/A on patients with mood disorders, with an emphasis on depression. The search was carried out in the PubMed, SCIELO, Web of Science and LILACS databases, using DeCS/MeSH descriptors, and included articles published in Portuguese and English between 2003 and 2023. After applying the criteria, 13 articles were selected for analysis. Overall, BoNT/A was shown to be an alternative that significantly improved facial signs of depression when compared to placebo in the studies carried out. In most of the RCTs, BoNT/A was applied to the mimic muscles of the upper third of the face, in the glabellar area, which was related to the improvement of mood disorders targeted by facial feedback. Thus, clinical evidence suggests that the management of emotional disorders includes the use of botulinum toxin therapy.

Nesse sentido, as toxinas botulínicas apresentam um amplo espectro de indicações, tanto cosméticas, quanto terapêuticas. Dessa forma, este estudo teve como objetivo realizar uma pesquisa bibliográfica sobre a utilização da toxina botulínica Tipo A (BoNT/A) na estética facial e no processamento das emoções. Para isso, foram avaliados ensaios clínicos randomizados (ECRs) que analisassem os efeitos da BoNT/A em pacientes com transtornos de humor, com ênfase na depressão. A pesquisa foi realizada nos bancos de dados PubMed, SCIELO, Web of Science e LILACS, utilizando descritores DeCS/MeSH, tendo sido incluídos os artigos publicados em português e inglês entre os anos de 2003 a 2023. Após aplicação dos critérios, 13 artigos foram selecionados para análise. No geral, a BoNT/A mostrou-se como sendo uma alternativa para melhorar significativamente os sinais faciais de depressão quando comparado ao placebo nos estudos realizados. Na maioria dos ECRs a BoNT/A foi aplicada nos músculos mímicos do terço superior do rosto, na área glabelar, a qual esteve relacionada com a melhora dos transtornos de humor com alvo no feedback facial. Assim, evidências clínicas apontam para que o manejo dos distúrbios emocionais abranja a utilização da terapia com uso da toxina botulínica.La acción de las toxinas botulínicas es el resultado de su potente capacidad para inhibir la liberación de neurotransmisores. En este sentido, las toxinas botulínicas tienen un amplio abanico de indicaciones, tanto cosméticas como terapéuticas. El objetivo de este estudio fue realizar un estudio bibliográfico sobre el uso de la toxina botulínica tipo A (BoNT/A) en la estética facial y el procesamiento de las emociones. Para ello, se evaluaron ensayos clínicos aleatorizados (ECA) que analizaban los efectos de la BoNT/A en pacientes con trastornos del estado de ánimo, con énfasis en la depresión. La búsqueda se realizó en las bases de datos PubMed, SCIELO, Web of Science y LILACS, utilizando los descriptores DeCS/MeSH, e incluyó artículos publicados en portugués e inglés entre 2003 y 2023. Después de aplicar los criterios, se seleccionaron 13 artículos para el análisis. En general, BoNT/A se mostró como una alternativa que mejoró significativamente los signos faciales de la depresión en comparación con placebo en los estudios realizados. En la mayoría de los ECA, la BoNT/A se aplicó en los músculos mímicos del tercio superior de la cara, en la zona glabelar, lo que se relacionó con la mejora de los trastornos del estado de ánimo a los que se dirigía la retroalimentación facial.
