Contention therapy induced in manual function in patients affected by stroke: a systematic review

Resumo: This systematic review sought to analyze the influence of contention therapy induced (CTI) on manual function in patients with stroke. The PUBMED, BIREME and SciELO databases were used, with the descriptors: “Stroke” AND “Induced Strain Therapy” AND “Upper Limb”, and their respective correspondents in English. Articles in English and Portuguese, published between 2000 and 2014, which used CTI to improve the manual function of individuals affected by stroke were included. Literature review articles and articles repeated in the databases were excluded. The crossing of descriptors resulted in 380 studies and, after applying the eligibility criteria, 10 articles remained. Studies have found that CTI is able to promote cortical reorganization through intensive and repetitive training of the upper limb compromised by stroke. The authors observed that the technique contributed to improving the function of the compromised upper limb when the therapy was intensive, performed for long periods. The effectiveness of CTI was observed in the short and long term, as long as movements are avoided. Therefore, CTI is a good alternative for the rehabilitation of patients with stroke and who neglect the paretic limb, due to muscle weakness, alteration of tone and other associated factors.

Se utilizaron las bases de datos PUBMED, BIREME y SciELO, con los descriptores: "Stroke" Y "Terapia de deformación inducida" Y "Miembro superior", y sus respectivos corresponsales en inglés. Se incluyeron artículos en inglés y portugués, publicados entre 2000 y 2014, que utilizaron las TIC para mejorar la función manual de las personas afectadas por un accidente cerebrovascular. Se excluyeron los artículos de revisión de literatura y los artículos repetidos en las bases de datos. El cruce de descriptores resultó en 380 estudios y, después de aplicar los criterios de elegibilidad, quedaron 10 artículos. Los estudios han encontrado que las TIC pueden promover la reorganización cortical a través del entrenamiento intensivo y repetitivo de la extremidad superior comprometido por un accidente cerebrovascular. Los autores observaron que la técnica contribuyó a mejorar la función de la extremidad superior comprometida cuando la terapia fue intensiva, realizada durante largos períodos. La efectividad de las TIC se observó a corto y largo plazo, siempre que se eviten los movimientos. Por lo tanto, las TIC son una buena alternativa para la rehabilitación de pacientes con accidente cerebrovascular y que descuidan la extremidad parética, debido a la debilidad muscular, la alteración del tono y otros factores asociados.A presente revisão sistemática objetivou analisar a influência da terapia de contenção induzida (TCI) na função manual em pacientes acometidos por AVE. Foram utilizadas as bases de dados PUBMED, BIREME e SciELO, com os descritores: “Acidente Vascular Cerebral” AND “Terapia de Contensão Induzida” AND “Membro Superior”, e seus respectivos correspondentes em inglês. Foram incluídos artigos em inglês e português, publicados entre 2000 a 2014 e que utilizaram a TCI para melhora da função manual de indivíduos acometidos pelo AVE. Excluíram-se artigos de revisão da literatura e os artigos repetidos nas bases de dados. O cruzamento dos descritores resultou em 380 estudos e, após aplicação dos critérios de elegibilidade, permaneceram 10 artigos. Os estudos observaram que a TCI é capaz de promover reorganização cortical através do treino intensivo e repetitivo do membro superior comprometido pelo AVE. Os autores observaram que a técnica contribuiu para melhoria da função do membro superior comprometido quando a terapia foi intensiva, realizada por longos períodos. A eficácia da TCI foi observada em curto e longo prazo, desde que evitados movimentos.
