Epidemiological profile of congenital heart diseases in a maternity hospital in the Serra Catarinense in 2016

Resumo: Objectives: To identify the types of congenital heart diseases among newborns in the year 2016. Methods: A retrospective, quantitative and descriptive study was carried out based on the evaluation of medical records. The medical records of the patients who underwent transthoracic echocardiography performed in 2016 were analyzed on the type of heart disease, birth weight, gestational age, gender, and clinical outcome of newborns in the maternity ward in Santa Catarina State, from 0 to 28 days old. Results: Transthoracic echocardiograms revealed atrial septal defect, persistent ductus arteriosus, ventricular septal defect, transposition of large vessels at baseline, atrioventricular septal defect, tricuspid insufficiency, and coarctation of the aorta in descending order. The incidence of congenital heart disease in the sample collected was 14:1000. Prevalent gestational age was preterm, most of them with adequate weight for gestational age with low weight. Clinical outcome was hospital discharge in most cases. Conclusions: The profile of the cardiopathies, the predominance of low weight and the precocious gestational age point to the need for early diagnosis in order to avoid late repercussions that could be avoided through clinical follow - up and surgical interventions when necessary. The research points out the importance of the active search for congenital heart diseases in the neonatal period, suggesting the use of the fetal echocardiogram as an integral prenatal examination recommended by the Unified Health System, starting at 30 weeks of gestation.

Métodos: se realizó un estudio retrospectivo, cuantitativo y descriptivo basado en la evaluación de los registros médicos. Se analizaron los registros médicos de los pacientes que se sometieron a una ecocardiografía transtorácica realizada en 2016 sobre: tipo de enfermedad cardíaca, peso al nacer, edad gestacional, sexo y resultado clínico de los recién nacidos en el hospital de maternidad en la cordillera de Santa Catarina de 0 a 28 días de vida . Resultados: los ecocardiogramas transtorácicos realizados mostraron comunicación interauricular, persistencia del ductus arterioso, comunicación interventricular, transposición de grandes vasos de la base, defecto auriculoventricular septal, insuficiencia tricúspide y coartación de la aorta en orden decreciente. La incidencia de cardiopatía congénita en la muestra recogida fue de 14: 1000. La edad gestacional prevalente fueron los recién nacidos prematuros, la mayoría con peso adecuado para la baja edad gestacional. El resultado clínico fue el alta hospitalaria en la mayoría de los casos. Conclusiones: el perfil de las enfermedades cardíacas, el predominio de bajo peso y la edad gestacional temprana apuntan a la necesidad de un diagnóstico temprano para evitar repercusiones tardías que podrían evitarse mediante el monitoreo clínico y las intervenciones quirúrgicas cuando sea necesario. La investigación señala la importancia de la búsqueda activa de enfermedades cardíacas congénitas en el período neonatal, sugiriendo el uso de la ecocardiografía fetal como un examen integral en el período prenatal recomendado por el Sistema Único de Salud, después de 30 semanas de gestación.Objetivos: Identificar os tipos das cardiopatias congênitas entre os recém nascidos no ano de 2016.  Métodos: Foi realizado um estudo, retrospectivo, quantitativo e descritivo a partir da avaliação dos prontuários. Foram analisados os prontuários dos pacientes que realizaram ecocardiograma transtorácico realizado no ano de 2016, sobre: tipo de cardiopatia, peso ao nascimento, idade gestacional, sexo e desfecho clínico de recém nascidos na maternidade na serra catarinense com idade de 0 a 28 dias de vida. Resultados: Os ecocardiogramas transtorácico realizados apontaram comunicação interatrial, persistência do canal arterial, comunicação interventricular, transposição de grandes vasos da base, defeito septal atrioventricular, insuficiência tricúspide e coarctação da aorta em ordem decrescente. A incidência de cardiopatia congênita na amostra coletada foi 14:1000. A idade gestacional prevalente foram os pré termos, a maioria com peso adequado para a idade gestacional com baixo peso. O desfecho clínico foi alta hospitalar na maioria dos casos. Conclusões: O perfil das cardiopatias, o predomínio de baixo peso e a idade gestacional precoce apontam para a necessidade de diagnóstico precoce visando evitar repercussões tardias que poderiam ser evitadas através de acompanhamento clínico e de intervenções cirúrgicas quando necessário.
