Associated factors with pressure injury in elderly people with gastrointestinal cancer

Resumo: Objective: To evaluate the frequency of pressure injuries and associated factors in the older pacients with gastrointestinal cancer. Methods: Cross-sectional study of 212 individuals aged 60 years and older and diagnosed with untreated gastrointestinal cancer confirmed by histology, cytology or immunohistochemistry. Patients who had undergone previous cancer treatment, except surgical ones, were excluded. The variables studied were: pressure injury, nutritional status, weight loss in the three months prior to the assessment, functionality, level of physical activity, presence of comorbidities and laboratory variables. For media comparisons, Student's test and categorical variables or square test or Fisher's exact test were used. In the multivariate analysis using logistic regression, the Wald test was used to evaluate the independent effects of factors with p <0.20 and applied to the Hosmer-Lemesho test. Results: The frequency of LP was 68/212 (32.1%). In the multivariate analysis, variables that may remain associated with the presence of LP were: hypertension and cerebrovascular disease. The final model presents an adequate fit to the data (Hosmer-Lemeshow test (p = 0.736). Conclusion: these findings can guide future research and can serve as a guide in health care services decisions about the use of pressure injury prevention strategies.

Metodología: este es un estudio transversal, con um enfoque cuantitativo. Participaron 212 personas mayores on cáncer en el tracto gastrointestinal. Las variables estudiadas fueron: lesión por presión, estado nutricional, pérdida de peso en los tres meses previos a la evaluación, funcionalidad, nivel de actividad física, presencia de comorbilidades y variables de laboratorio. Para las comparaciones de medias, se utilizó la prueba t de Student y para las variables categóricas, la prueba de chi-cuadrado o la prueba exacta de Fisher. En el análisis multivariante, mediante regresión logística, se utilizó la prueba de Wald para evaluar los efectos independientes de los factores con un valor de p<0,20 y se aplicó la prueba de Hosmer-Lemeshow. Resultados: la frecuencia de las lesiones por presión fue del 32,1%, considerada un valor alto para este resultado. En el análisis multivariante, las variables que permanecieron asociadas a la presencia de lesiones por presión fueron: hipertensión y enfermedad cerebrovascular. El modelo final presentó un ajuste adecuado a los datos (p= 0.736). Conclusión del estudio: estos hallazgos pueden guiar la investigación futura y pueden servir como guía para tomar decisiones con respecto al uso de estrategias de prevención de lesiones por presión.Objetivo: avaliar a frequência de lesões por pressão e os fatores associados em idosos com câncer no trato gastrointestinal. Resultados: A frequência de lesão por pressão foi de 32,1%, considerado um valor alto para este desfecho.  Na análise multivariada, as variáveis que permaneceram associadas a presença de lesão por pressão foram: hipertensão e doença cerebrovascular. O modelo final apresentou um ajuste adequado aos dados (p=0,736).
