Relation between acute pancreatitis and diabetic emergencies: a literature review

Resumo: Acute pancreatitis (AP) is an inflammatory disease that affects the pancreas and peripancreatic tissues, while diabetic keatoacidosis (DKA) is a metabolic disorder resulting from a hyperglycemic state, with a state of metabolic acidosis and ketosis. About 77% of patients with DKA have signs of pancreatic disease. The objective of this study is to analyze the correlation of AP with diabetic emergencies and encourage medical professionals to explore this topic in view of the growing number of AP cases associated with DKA. This is an integrative literature review carried out through electronic searches in the following databases: PubMed, Scielo and Lilacs in March 2022. In these searches, the following descriptors were applied together with the Boolean operator AND: Pancreatitis AND Hyperosmolar Hyperglycemic State AND Diabetic Ketoacidosis, Pancreatitis AND Hyperosmolar Hyperglycemic State and Pancreatitis AND Diabetic Ketoacidosis. The guiding question to guide the correct search for articles was “What is the relation between acute pancreatitis and diabetic emergencies?” The research was carried out from a review of published studies on the correlation of DKA or hyperosmolar hyperglycemic state (HHS) with AP, and the association between cause and effect. In addition, most articles showed the development of hypertriglyceridemia (HTG) as a consequence of this hyperglycemic state. The reports follow two pathophysiological biases, the first correlating ketoacidosis as the cause of acute pancreatitis, and the second based on acute pancreatitis as the cause of diabetic ketoacidosis. It is concluded that the relationship between AP and diabetic emergencies appears in different ways. There are cases in which pancreatic inflammation is responsible for complications and cases in which it is a consequence of these. Hypertriglyceridemia is also present and can often be the cause of acute pancreatitis in conjunction with ketoacidosis.

Alrededor del 77% de los pacientes con CAD tienen signos de enfermedad pancreática. El objetivo de este estudio es analizar la correlación de PA con emergencias diabéticas y alentar a los profesionales médicos a explorar este tema en vista del creciente número de casos de PA asociados con CAD. Esta es una revisión integrativa de la literatura realizada a través de búsquedas electrónicas en las siguientes bases de datos: PubMed, Scielo y Lilacs en marzo de 2022. En estas investigaciones se aplicaron los siguientes descriptores junto con el operador booleano AND: Pancreatitis AND Hyperosmolar Hyperglycemic State AND Diabetic Ketoacidosis, Pancreatitis AND Hyperosmolar Hyperglycemic State and Pancreatitis AND Diabetic Ketoacidosis. La pregunta orientadora para orientar la búsqueda correcta de artículos fue “¿Cuál es la relación entre la pancreatitis aguda y las emergencias diabéticas?” La investigación se realizó a partir de una revisión de estudios publicados sobre la relación de la CAD o estado hiperglucémico hiperosmolar (EHH) con la PA, y la asociación entre causa y efecto. Además, la mayoría de los artículos mostraron el desarrollo de hipertrigliceridemia (HTG) como consecuencia de este estado hiperglucémico. Los informes siguen dos sesgos fisiopatológicos, el primero que correlaciona la cetoacidosis como la causa de la pancreatitis aguda y el segundo basado en la pancreatitis aguda como la causa de la cetoacidosis diabética. Se concluye que la relación entre AF y urgencias diabéticas se presenta de diferentes formas. Hay casos en que la inflamación pancreática es responsable de las complicaciones y casos en que es consecuencia de estas. Cerca de 77% dos pacientes com CAD possuem sinais de doença pancreática. O objetivo deste estudo é analisar a correlação da PA com as emergências diabéticas e estimular os profissionais da área médica a explorar essa temática diante do crescente números de casos de PA associados a CAD. Trata-se de uma revisão integrativa da literatura realizada por meio de pesquisas eletrônicas nas seguintes bases de dados: PubMed, Scielo e Lilacs no mês de março de 2022. Nessas pesquisas, foram aplicados os seguintes descritores em conjunto com o operador booleano AND: Pancreatitis AND Hyperosmolar Hyperglycemic State AND Diabetic Ketoacidosis, Pancreatitis AND Hyperosmolar Hyperglycemic State e Pancreatitis AND Diabetic Ketoacidosis. A pergunta norteadora para guiar a busca correta dos artigos foi “Qual a relação da pancreatite aguda com as emergências diabéticas?” A pesquisa foi realizada a partir de uma revisão de estudos publicados sobre a relação da CAD ou estado hiperosmolar hiperglicêmico (EHH) com a PA, e a associação entre causa e efeito. Além disso, a maioria dos artigos evidenciaram como consequência desse estado hiperglicêmico, o desenvolvimento da hipertrigliceridemia (HTG). Os relatos seguem dois vieses fisiopatológicos, sendo o primeiro correlacionando cetoacidose como causa da pancreatite aguda, e o segundo fundamentado na pancreatite aguda como causa da cetoacidose diabética. Conclui-se que a relação entre PA e emergências diabéticas se apresenta de diversas maneiras. Há casos em que a inflamação pancreática é responsável pelas complicações e casos em que é consequência destas.
