Association between hypertriglyceridemic waist phenotype and Diabetes mellitus in adults: a case-control study

Resumo: | Diabetes mellitus (DM) is considered a major public health problem. Studies in several countries points to the usefulness of evaluating the hypertriglyceridemic waist (HTC) phenotype to predict the risk of the disease. Aim: This study aimed to evaluate the association between the HTC phenotype and DM type 2 in Dourados, MS, Brazil. Methods: This is a case-control study nested within a population-based cross-sectional survey with multiple-stage sampling, carried out in 2016. The sample included 293 adults aged 20 to 59 years, of which 56 were cases and 237 controls. The cases were those with fasting blood glucose ≥ 126mg/dL and who reported that “a doctor or health professional has already said that “Do you have high blood sugar?” Controls were drawn from the other interviewees, without DM. Individuals with waist circumference (WC) were considered to have the HTC phenotype: ≥ 80 cm for women and ≥ 90 cm for men, combined with hypertriglyceridemia (TG) ≥ 150 mg/dL. The variables gender, age, education, socioeconomic status, smoking, alcohol consumption, physical activity and body mass index were investigated. Chi-Square Test, Fisher's Exact Test and Unconditional Logistic Regression were used for data analysis. Results: The HTC phenotype was positively associated with DM: 2.08, 95%CI: 1.04-4.12, after adjusting for covariates. Conclusion: The study suggests that the HTC phenotype may be useful as an approach in the identification of DM. |

Estudios en varios países apuntan a la utilidad de evaluar el fenotipo de cintura hipertrigliceridémica (CHT) para predecir el riesgo de la enfermedad. Objetivo: Este estudio tuvo como objetivo evaluar la asociación entre el fenotipo HTC y DM tipo 2 en Dourados-MS, Brasil. Métodos: Se trata de un estudio de casos y controles anidado dentro de una encuesta transversal de base poblacional con muestreo polietápico, realizada en 2016. La muestra estuvo compuesta por 293 adultos de 20 a 59 años, de los cuales 56 fueron casos y 237 controles. Los casos fueron aquellos con glucemia en ayunas ≥ 126mg/dL y que reportaron que “algún médico o profesional de la salud ya les dijo que el Sr. ¿Tienes un nivel alto de azúcar en la sangre?” Los controles se extrajeron de los otros entrevistados, sin DM. Se consideró que los individuos con circunferencia de cintura (CC) tenían el fenotipo HTC: ≥ 80 cm para mujeres y ≥ 90 cm para hombres, combinado con hipertrigliceridemia (TG) ≥ 150 mg/dL. Se investigaron las variables sexo, edad, escolaridad, nivel socioeconómico, tabaquismo, consumo de alcohol, actividad física e índice de masa corporal. Para el análisis de los datos se utilizaron la prueba de chi-cuadrado, la prueba exacta de Fisher y la regresión logística incondicional. Resultados: El fenotipo CHT se asoció positivamente con DM: 2,08, IC 95%: 1,04-4,12, después de ajustar por covariables. Métodos: Trata-se de um estudo de casos e controles aninhado a uma pesquisa transversal de base populacional com amostragem em múltiplos estágios, realizado em 2016. A amostra incluiu 293 adultos de 20 a 59 anos, dos quais 56 casos e 237 controles. Os casos foram aqueles com glicemia de jejum ≥ 126mg/dL e que referiram que “algum médico ou profissional de saúde já disse que o Sr. tem açúcar alto no sangue?”. Os controles foram sorteados dos demais entrevistados, sem DM. Consideraram-se com presença do fenótipo CHT indivíduos que apresentaram circunferência da cintura (CC): ≥ 80 cm para mulheres e ≥ 90 cm para homens, combinada com hipertrigliceridemia (TG) ≥ 150 mg/dL. Foram investigadas as variáveis sexo, idade, escolaridade, estrato socioeconômico, tabagismo, consumo de bebida alcóolica, atividade física e índice de massa corporal. Teste Qui-Quadrado, Teste Exato de Fisher e Regressão Logística Não Condicional foram utilizados para análise dos dados. Resultados: O fenótipo CHT apresentou-se positivamente associado ao DM: 2,08, IC95%: 1,04-4,12, após o ajuste para covariáveis.
