Comparative study between the influence of fixed and mobile orthodontic appliances on the oral microbiota

Resumo: Orthodontic appliances fixed and mobile are used for the treatment of malocclusion, dental corrections, problems in arch development and growth and for aesthetic reasons. Therapies performed with these devices can affect the oral microbiota resulting in a series of effects on oral health. There is a large discrepancy when we compare the types of appliances and their implications on the oral microbiota, with fixed appliances being the ones that affect the most, by the difficulty of cleaning they provide and by the bigger force exerted on the periodontium. This literature review aims to detail the secondary effects of the use of orthodontic appliances in the constitution of the oral microbial community. This is a descriptive bibliographic study of the narrative review type. Using the descriptors "Orthodontic Appliances, Fixed", "Orthodontics", "Microbiota" and "Periodontal Diseases", a search was performed on the digital platforms VHL, Capes, SciELO and PubMed using the Boolean operators “AND” and “OR”. From a total of 31 works 6, 29 were selected in Portuguese, English and Spanish, dating between 2014 and 2022. It was found that by the difficulty in cleaning and the greater force exerted on the periodontium results in changes in the growth rate of certain species of bacteria and fungi, being variable according to the time of use, presence of biofilm, development of caries and involvement of periodontal diseases.

Las terapias realizadas con estos dispositivos pueden afectar a la microbiota bucal, lo que se traduce en una serie de efectos sobre la salud bucal. Existe una gran discrepancia cuando comparamos los tipos de aparatología y sus implicaciones sobre la microbiota bucal, siendo la aparatología fija la que más afecta, por la dificultad de limpieza que aportan y la mayor fuerza que ejercen sobre el periodonto. Esta revisión de la literatura tiene como objetivo detallar los efectos secundarios del uso de aparatos de ortodoncia en la constitución de la comunidad microbiana bucal. Se trata de un estudio bibliográfico descriptivo del tipo revisión narrativa. Utilizando los descriptores “Aparatos de Ortodoncia Fijo”, “Ortodoncia”, “Microbiota” y “Enfermedades Periodontales”, se realizó una búsqueda en las plataformas digitales BVS, Capes, SciELO y PubMed utilizando los operadores booleanos “AND” y “OR”. De un total de 296, se seleccionaron 31 en portugués, inglés y español, con fechas entre 2014 y 2022. Se encontró que debido a la dificultad en la limpieza que proporcionan y la mayor fuerza que ejercen sobre el periodonto, resultando en cambios en la tasa de crecimiento de ciertas especies de bacterias y hongos, siendo variable según el tiempo de uso, presencia de biopelícula, desarrollo de caries y afectación de enfermedades periodontales.Os aparelhos ortodônticos tanto fixos como os móveis são utilizados para o tratamento de maloclusão, correções dentárias, problemas no desenvolvimento dos arcos e no crescimento e para razões estéticas. As terapias realizadas com esses aparatos podem afetar a microbiota oral, o que resulta em uma série de efeitos na saúde bucal. Existe uma grande discrepância quando comparamos os tipos de aparelhos e as suas implicações na microbiota bucal, sendo os aparelhos fixos os que mais afetam, pela dificuldade de higienização que eles proporcionam e pela maior força exercida no periodonto. Esta revisão de literatura objetiva detalhar os efeitos secundários do uso de aparelhos ortodônticos na constituição da comunidade microbiana bucal. Trata-se de um estudo bibliográfico descritivo do tipo revisão narrativa.
