Surgical treatment of mandibular osteoma – case report

Resumo: Osteomas are benign neoplasms that are characterized by the proliferation of mature compact or spongy bone. The most frequent sites of occurrence are the mandible and paranasal sinuses. The purpose of this article is to report the diagnosis and surgical treatment of a mandibular peripheral osteoma in a non-syndromic patient. Male patient, 40 years old, ASA I, sought the service of Maxillofacial Surgery at Hospital Universitário Oswaldo Cruz - PE with a complaint of increased intraoral volume in the anterior region of the mandible, firm palpation, well delimited and about 2 cm. After physical examination, the diagnostic hypothesis raised was osteoma and mandibular torus. The planned surgical proposal was intra-oral access, excision of the lesion, mandibular osteoplasty and referral of the material collected for histopathological analysis to confirm the diagnosis. Histopathological analysis confirmed the diagnostic hypothesis of peripheral osteoma. The patient was followed up in the postoperative period and after 6 months of the surgical procedure, no signs of recurrence were observed.

Los sitios más frecuentes de aparición son los senos mandibulares y paranasales. El propósito de este artículo es informar el diagnóstico y el tratamiento quirúrgico de un osteoma periférico mandibular en un paciente no sindrómico. Paciente de sexo masculino, 40 años, ASA I, buscó el servicio de Cirugía Maxilofacial en el Hospital Universitario Oswaldo Cruz - PE con una queja de aumento del volumen intraoral en la región anterior de la mandíbula, palpación firme, bien delimitada y de unos 2 cm. Después del examen físico, la hipótesis diagnóstica planteada fue osteoma y toro mandibular. La propuesta quirúrgica planificada fue el acceso intraoral, la escisión de la lesión, la osteoplastia mandibular y la derivación del material recolectado para el análisis histopatológico para confirmar el diagnóstico. El análisis histopatológico confirmó la hipótesis diagnóstica del osteoma periférico. El paciente fue seguido en el postoperatorio y después de 6 meses del procedimiento quirúrgico, no se observaron signos de recurrencia.Osteomas são neoplasias benignas que se caracterizam pela proliferação de osso maduro compacto ou esponjoso. Os locais de ocorrência mais frequentes são a mandíbula e os seios paranasais. O objetivo desse artigo é relatar diagnóstico e o tratamento cirúrgico de um osteoma periférico mandibular em paciente não sindrômico. O caso em estudo é o de um Paciente do gênero masculino, 40 anos, ASA I, procurou o serviço de Cirurgia Bucomaxilofacial do Hospital Universitário Oswaldo Cruz – PE com queixa de aumento de volume intraoral em região anterior de mandíbula, firme a palpação, bem delimitada e com cerca de 2cm. Após exame físico a hipótese diagnóstica levantada foi osteoma e torus mandibular. A proposta cirúrgica planejada foi acesso intra-oral, exérese da lesão, osteoplastia mandibular e encaminhamento do material colhido para análise histopatológica para confirmação do diagnóstico. A análise histopatológica confirmou a hipótese diagnóstica de osteoma périférico.
